Age-related Changes in Anterior, Posterior Corneal Astigmatism in a Korean Population

목적: 한국인에서 연령 증가에 따른 각막 전면 및 후면 난시의 변화를 알아보고자 하였다. 대상과 방법: 11세부터 92세까지 특이 안질환이 없는 160명의 각막 전면 및 후면 난시를 회전 샤임플러그 카메라(Pentacam &#9415;)를 이용 하여 측정하였다. 이 자료를 이용해 연령 증가에 따른 각막 전면 및 후면난시에서 도난시와 직난시의 비율변화와 극값을 이용한 변화 경향을 분석하였다. 결과: 각막 전면 난시는 연령이 증가함에 따라 직난시의 비율이 감소하는 경향을 보였으며( p <0.0001), 각막 후면 난시는 도난시의 비율이 감소하는 경향을 보였다( p =0.012). 극값을 이용하여 분석한 결과에서는 연령이 증가함에 따라 각막 전면 난시는 도난시로 변 화하는 경향( p <0.0001)을, 후면 난시는 직난시로 변화하는 경향을 보였다( p <0.0001). 결론: 기존 연구들에서 각막 전면 난시는 연령이 증가함에 따라 직난시에서 도난시로 변화하는 경향을 보이는 반면, 각막 후면 난시는 연령과 관계없이 대부분 도난시인 모습을 보였다. 그러나 본 연구결과에 따르면 각막 후면 난시는 연령이 증가함에 따라 도난시의 비율이 감소하는 경향을 보였다. 따라서 고령인 환자에서의 백내장수술 시, 특히 다초점 혹은 난시교정 인공수정체를 사용할 경우 각막후면 난시에 대한 평가를 반드시 시행해야 한다.

Purpose: To evaluate age-related changes in anterior and posterior corneal astigmatism in Koreans. Methods: The anterior and posterior corneal astigmatisms of 160 subjects with age ranging from 11 to 92 years, none of whom experienced any complications, were measured with a rotating Scheimpflug camera (Pentacam ？？ ). Using this data, the changing proportions of with-the-rule to against-the-rule and changing tendency of anterior and posterior corneal astigmatisms with age were evaluated using polar value analysis according to the Naeser method. Results: For the anterior cornea, the proportion of with-the-rule astigmatisms decreased with age (p < 0.0001). On the other hand, for the posterior cornea, the proportion of against-the-rule astigmatisms decreased with age (p = 0.012). In the polar value analysis, there was a trend toward against-the-rule astigmatism associated with increasing age for the anterior cornea (p < 0.0001) and toward with-the-rule astigmatism for the posterior cornea (p < 0.0001). Conclusions: In previous studies, the anterior corneal surface shifted from with-the-rule to against-the-rule astigmatism with in-creasing age, whereas the posterior corneal surface remained as against-the-rule astigmatism in most cases. But, our results showed that the proportion of against-the-rule astigmatisms of the posterior cornea decreased with age. Thus, evaluation of pos-terior corneal astigmatisms should be performed before cataract surgery in old patients, especially when using a multifocal or toric intraocular lens.
