의료서비스 이용에서의 차이가 주관적 건강상태의 불평등에 미치는 영향:

The impact of health care service utilization on disparities in self-rated health:

본 연구의 목적은 의료서비스 이용수준에 미치는 요인들을 앤더슨 모형의 선행·가능·욕구요인들로 살펴보고, 의료서비스 이용에서의 차이가 주관적 건강상태에서의 불평등을 설명하는 매개요인으로서 작용하는지를 조사하였다. 본 연구의 분석을 위해 2012년도 7차 한국복지패널에서 제공하는 가구용 데이터와 가구원용 데이터가 통합된 데이터에 포함된 14,604명을 연구대상으로 삼았다. 주관적 건강상태에서의 불평등에 영향을 미치는 다원적 요인들의 효과를 동시에 분석하기 위하여 구조방정식 모형을 이용하여 분석하였다. 분석의 결과에 따르면 앤더슨 모형의 선행요인 가운데 연령, 성별, 교육수준 모두 의료서비스 이용수준과 관련이 있었다. 가능 요인들 중에서는 소득이 높고 민간의료보험을 많이 가지고 있는 경우에 건강검진 이용횟수가 높은 것으로 나타났다. 욕구요인에서는 오래기간 만성질환이 있거나 장애가 있는 경우에 입원 서비스 이용횟수가 높게 나타났다. 마지막으로 의료서비스 이용수준은 외래, 입원, 건강검진 서비스 형태별로 선행·가능·욕구 요인들과 주관적 건강상태를 매개하는 효과가 각각 다르게 나타났는데, 본 연구에서는 다중매개분석을 통하여 구체적인 매개효과의 통계적 유의성을 검증하 였다. 본 연구의 결과를 토대로 의료서비스 이용의 형평성을 증진시키고, 주관적 건강상태에서의 불평등을 줄이기 위한 대안이 논의되었다.

The purposes of this study is to examine predisposing·enabling·needs factors which impact on disparities in health care service utilization, and to test their mediating effects on disparities in self-rated health. For data analyses, 14,604 people who participated in the seventh wave of Korean welfare panel(2012) were used. Structural equation modeling was utilized to analyze the impact of various factors on disparities in self-rated health simultaneously. The results showed that among predisposing factors, age, gender, and levels of education were significantly associated with disparities in health care service utilization. Among enabling factors, higher levels of income and having more private insurances significantly predicted increased number of medical examinations. Need factors such as having chronic diseases and disability significantly predicted increased uses of both outpatient and inpatient services. Finally the mediating effect of health care service utilization between predisposing·enabling·needs factors and self-rated health differ by its service type such as outpatient, inpatient, and medical examination. This study examined a specific indirect effect of each health care service through multiple mediation analyses. Based the results of this study, it was discussed how to improve equality in health care service uses and to minimize disparities in self-rated health.
