정신분열병 환자의 사회적응력 향상을 위한 문제해결기술훈련의 적용에 대한 연구

A Study on the Use of Problem-Solving Skill Training for Social Adjustment of Schizophrenic Patients

본 연구는 정신분열병 환자의 지역사회 적응을 위해 사회기술훈련을 통한 스트레스 대처기술과 사회적 유능감의 습득이 중요하다는 논의에서 출발하였으며, 사회기술훈련의 전략으로서, 이들의 질병특성인 인지기능의 손상을 고려한 문제해결접근을 제안하고자 하였다. 이를 위해 우선, 문제해결기술훈련의 개념과 단계에 대해 알아보고, 정신분열병 환자의 문제해결능력에 대한 연구결과들을 고찰하였으며, 이를 바탕으로 보다 효과적인 문제해결기술훈련을 위한 구체적인 개입장법을 제시하여 낮치료 프로그램에 다니고 있는 정신분열병 환자를 대상으로 3개월간 수행하였다. 특히, 기존의 문제해결기술훈련이 일반적인 대인관계 상황을 주로 다루었던 것과 달리, 본 연구에서는 화자들이 훈련 프로그램의 참여에 대한 목적의식을 분명히 갖도록 하기 위해 직업생활영역과 같이 보다 구체적인 생활영역에서의 문제해결기술의 습득에 중점을 두었다. 수행된 프로그램의 결과를 평가하기 위하여 실험/비교 사전-사후 실험설계를 사용하였다. 집단점수의 분석결과, 실험집단의 문제해결기술인 비교집단보다 훈련전ㆍ후에 유의미하게 향상되었음을 보여주었고, 개인사례의 분석결과, 문제해결기술훈련의 적용대상에 대한 고려의 필요성이 제기되었다. 연구결과의 분석을 토대로, 정신분열병 환자의 지역사회 생활준비를 위해 보다 다양한 생활영역에서의 적응을 위한 개입방법으로서 문제해결기술훈련을 활용할 수 있을 것임을 제안하였다.

It has become evident that social skills training plays an important role in social adjustment of schizophrenic patients by improving their social competence and ability to cope with stressors. This study suggested the problem-solving approach as an effective social skills training model for schizophrenic patients. Based on the literature reviews of problem-solving skill, a specific problem-solving skill training program on the vocational area was developed and provided for schizophrenic patients. The effects of intervention concerned with enhancing the job related problem-solving skills were examined using experimental/control pretest-posttest design. The data provided useful information about the potential role of the training on the job related problem-solving skill by showing that the results of experimental group were superior to those of a control group. Finally, applicability of the problem-solving skill training for schizophrenic patients in a variety of community living situations was discussed.
