노인요양욕구사정도구에 관한 국가 비교 연구

An International Comparative Study on Elderly Need Assessment and Classification Instrument

본 연구는 2008년 도입될 노인장기요양보장제도의 근간이 되는 대상자 등급판정에 대한 기준을 국가간 비교를 통해 고찰해 봄으로써, 우리나라 노인의 장기요양욕구에 적절히 대처할 수 있는 기준을 마련하기 위해 논의되어야 할 점을 살펴보는데 목적이 있다. 본 연구는 공적 요양보장체계를 갖추고 있는 일본, 독일을 비롯하여, 공적 요양보장체계는 아니지만, 수가를 통해 요양 욕구를 지원하고 있는 미국, 호주, 그리고 그 외 노인의 기능상태별로 차등지원하고 있는 영국과 싱가폴의 노인장기요양선정기준을 분석하였다. 연구결과 무엇보다도 각 나라에서는 각 나라의 요양제도에 따라 대상자 선정도구의 목적과 사용을 달리 하고 있다는 것을 알 수 있었다. 즉 장기요양욕구를 위해 공적제도를 갖추고 있는 일본과 독일과 같은 나라는 요양욕구를 위한 등급판정도구로써 사용하고 있으며 따라서 등급판정도구는 등급에 영향을 주는 매우 간명한 항목으로 구성하였다. 호주와 싱가폴은 시설입소노인을 지원하기 위하여 기능상태에 따라 차등지원하는 도구로 쓰이고 있으며, 등급판정도구와 요양계획을 위한 도구는 별도로 존재한다. 미국과 영국은 주별로 차이로 보이지만, 일차적으로 요양계획을 위한 도구가 공식화 되어 있으며 이 중 차등지원을 위한 항목이 별도로 존재한다. 그러나 각 나라는 요양욕구에 대한 다양한 제도를 선택하고 이에 따라 요양요구 사정도구의 쓰임새를 달리하고 있지만, 공통된 논리를 가지고 있다. 즉 요양욕구를 급여와 관련하여 차등지원할 때 서비스 시간이라고 조작화 될 수 있는 서비스 필요도(자원 사용량)에 의해 구분되도록 그 기준이 개발되어졌다. 이상의 연구결과를 통해 우리나라 노인장기요양보험제도의 요양대상 사정도구에서 향후 고려되어져야 할 점으로는 첫째, 도구의 목적과 활용방안에 대한 사회적 공유가 필요하며, 둘째, 등급판정도구와 욕구사정도구 및 케어플랜과의 관계를 정립할 필요가 있다. 셋째, 노인욕구사정도구의 자율화와 표준화에 따른 장단점을 파악하여 제도의 효율성과 비용효과면에서 적정수준에서 결정될 필요가 있으며, 넷째, 노인요양대상자 선정을 위한 평가판정체계의 확립을 위한 논의들이 이루어져야 할 것이다.

The purpose of this study is to give implications to develope Korean Elderly Long-Term Care Assesment and Oassification instrument by analyzing foreign instrument comparatively. This study analyzed long- term care assessment and classification instrument on Japan, Germany, United State, Austria, United Kingdom and Singapore. Throughout object of countries, these are founded. First of all, it is different that the purpose and using of instrument is adopted in each system in each country. Some countries adopt public system for long term care(ex, Japan, Germany, and Australia), they use instrument as classification just for care needs so that instrument is composed parsimonious items. Other countries that have not public system for long term care adopt instrument as comprehensive needs assessment about clinical function. But they have common logic throughout every counties, when they related to benefit, the instruments are developed by service need(resource utilization) that operationalize service(care) time. Considering points in developing Korean Elderly long-Term Care Assesment and Oassification instrument are First, it needs to be get identify the purpose of instrument and to have social understanding for instrument utilization, Second it needs to be taken a triangular position about relationship among classification of grade, need assessment for functional status, care plan and reimbursement. Third it needs to be decided to adequate level of efficiency and effectiveness about using compulsive or self-regulation instrument, and Forth, related to instrument, it should be discussed about long term care assessment and classification system. Finally, Korean instrument should to being consider not only assessing needs of long term care but also evaluating clinical function, care plan and benefit.
