노인의 증상경험에 영향을 미치는 요인

Factors affecting symptom experience of the elderly:based on the stress and health behavior

노인의 증가추세에 따라 노인의 증상경험에 대해 그 빈도와 강도, 고통감 등 다양한 차원에서의 평가 및 관리가 중요하다고 할 수 있다. 그런데 노인이 호소하는 증상이나 경험에 대한 선행연구는 찾아보기 어려웠고, 다양한 관련요인에 대한 연구는 거의 다루어지지 않았다. 따라서 본 연구에서는 노인의 다차원적인 증상경험(빈도, 강도, 고통감)을 확인하고 일반적 특성과, 스트레스 및 건강행위와의 관련성을 확인하고자 65세 이상 노인 123명을 대상으로 면담을 통한 설문조사를 실시하였다. 연구결과, 증상경험 차원 중빈도보다 강도와 고통감 모두가 낮은 경우, 빈도에 비해 고통감이 높은 경우, 빈도에 비해 강도와 고통감 모두가 높은 경우, 빈도나 고통감보다 강도가 높은 경우의 네 가지 유형으로 나타났다. 일반적 특성에 따른 증상경험정도는 직업, 수입에 대한 만족도, 주관적 건강상태 그리고 만성병 개수에서 유의한 차이를 보였다. 또한 제 변수들간의 관계분석시 스트레스는 증상경험과 순상관관계가 있었고, 건강행위는 증상경험과 역상관관계가 있었다. 마지막으로 증상경험의 주된 영향요인은 건강행위였고 스트레스, 주관적 건강상태 및 만성병을 포함하여 33.6%의 설명력을 보였다. 이상의 결과는 건강행위 이행 수준을 높이고 스트레스 수준을 낮추는 전략을 포함하여 노인의 증상관리를 위한 기초자료로 활용될 수 있을 것이다.

The purpose of this study was to evaluate factors affecting symptom experience(frequency, intensity, distress) of the elderly based on the stress and health behavior. Data were collected from 123 elderly using questionnaires through interviews. The results revealed significant differences between general characteristics and symptom experience, which of them were occupation, income, subjective health condition, and number of chronic disease. In terms of symptom experiences, musculoskeletal symptoms(2 items of the 25 symptoms) were identified as the most frequent, severe, and distressful. Also, symptom experience had a significant positive correlation with stress and negative correlation with health behavior. The findings of regression analysis indicated that compliance of health behavior, subjective health condition, chronic disease and stress influenced symptom experience of the elderly(R2=.33.6%). This study will be basic resources for systemic symptom management of the elderly including strategies to improve compliance of health behavior and decrease perceived stress.
