Endoscopic sphincterotomy is a difficulty in being taken in case of periampullary diverticulum during endoscopic retrograde cholangiopancreatography for stone removal. We tried removal of common bile duct stones by the endoscopic procedure with basket after minor endoscopic sphincterotomy and large balloon sphincterotomy with 20mm diameter. We report one case of this common bile duct stone：the stones were successfully removed by endoscopic procedure with basket and mechanical lithotripsy.
