우리나라 노인장기요양보험 등급판정기준에 대한 등급판정위원의 인식조사

The Perception Types of Classification Committee on the Long- Term Care Classification Criteria for Elderly People - Application of Q-methodology

본 연구는 노인장기요양보험 등급판정위원을 대상으로 Q방법론을 적용하여 등급판정의 주요요인에 대한 인식유형을 나누어 보고, 각 인식유형의 특성을 살펴보고자 함을 목적으로 한다. 연구결과 등급판정에 있어 의사소견서와 전문가적 판단을 중요하게 판단하는 “의료정보중시형”과 행동변화, 방임 및 학대, 정신과적 진단 등에 대한 요인을 중요하게 판단하는 “심리행동변화중시형”, 평가항목의 다양화, 의사소견서의 신뢰도 강화, 방문조사원의 전문성 강화, 등급판정위원회에 있어 심층면접과 관찰면접 보완 등에 대한 객관적 방법론을 추구하는 “객관적 방법론 중시형”, 가족수발자의 정서적 부담, 기존 서비스 이용내용, 개인 및 보호자의 생활정보 등을 중요하게 고려하는 “개별화 욕구 중시형”으로 유형화할 수 있었다. 이러한 연구결과는 그동안 실증적으로 검증되지 못했던 등급판정위원의 다양한 판정기준의 실체를 파악해 보았다는데 의의가 있으며 향후 등급판정 관련 정책결정과 집행과정에 있어 기초자료가 될 수 있다.

The purpose of this study is to find out perception types of classification committee who are belong to national long-term care insurance system. For this purpose, this study adopted Q-methodology which is developed mainly to study on the subjective conceptualization of related people on research theme. This study found out four major types of perception on classification criteria for long-term care needs, which are the types of focusing specialization and professionalization of clinical care diagnosis, valuing on psycho-behavioral factors, requesting objective information procedure, insisting on involvement of family care burden and environment factors of long-term care needs. This research findings can be used to make clear understanding on diverse voices of classification committee about long-term care need criteria for elderly people. This kind of understanding would contribute to mutual understanding and clear communication to arrive positive consensus related to the matters of long-term care needs for the elderly people.
