여성 암생존자의 정신건강 예측요인

Predictors of Mental Health in Female Cancer Survivors

우리사회의 암생존자 증가로 이들에 대한 지속적 관리의 중요성이 부각되고 있다. 특히 의료의 접근성 및 사회적 지지 체계의 미비로 인해 어려움을 겪게 되는 여성 암생존자에 대한 지원의 필요성이 제기되고 있다. 이들 여성 암생존자의 경우 우울, 불안, 자살사고 등의 정신건강상문제가 두드러지게 나타나고 있는데 이는 전반적인 삶의 질 저하뿐만 아니라 생존율에까지 부정적 영향을 초래하는 것으로 이해되고 있다. 이에 본 연구는 여성 암생존자의 정신건강에 영향을 미치는 요인을 검증하여 이들을 위한 사회적 지원책 마련에 기초자료를 제공하고자 수행되었다. 이를 위해 국민건강영양조사 제 6기자료 중 1차년도 자료인 2013년도 자료를 활용하여 125명의 여성 암생존자의 정신건강을 조사하였고 다중회귀분석을 통해 인구사회학적 요인과 질병관련 요인들의 영향력을 검증하였다. 분석결과, 여성 암생존자의 정신건강에 대해 인구사회학적 요인 중 의료급여 수급여부가 유의미한 영향을 나타내고 있었으며(β＝－.201, p＜.05). 의료급여 대상자인 저소득층의 여성 암생존자의 정신건강이 좋지 못하였다. 질병관련 요인에서는 주관적 건강인식 정도가 긍정적일수록 정신건강도 유의하게 좋은 것으로 나타났다(β＝－.236, p＜.05). 이러한 결과를 토대로 여성 암생존자의 정신건강 증진을 위한 심리사회적 개입 및 정책에 대한 제언을 제시하였다.

With increasing survival rates of cancer, cancer survivorship care plan becomes a more important issue. According to literatures, metal health of female cancer survivors is poorer than male and social interventions have been needed for them. However, little was known about the mental health of female cancer survivors. The purpose of this study was to identify predictors of mental health and to provide the framework for intervention to prevent the problem of mental health in female cancer survivors. The research was conducted with 125 data from the 2013 Korea National Health and Nutrition Examination Survey. Data were analyzed by t－test, ANOVA, hierarchical multiple regression analysis to identify significant factors and their effects on mental health of female cancer survivors. Significant factors were medical aid beneficiaries and selfperception of health. Mental health of female cancer survivors received medical aid was poorer than national health insurance significantly. Self－perception of health had a positive effect on mental health. Based on these results, the practical implications for mental health of female cancer survivors had been discussed.
