Purpose To evaluate the diagnostic accuracy of renal CT angiography in the preoperative evaluation of the renal transplant donors Materials and Methods From May 2001 to April 2004, 30 consecutive living kidney donors(17 men, 13 women; mean age, 39 years) who underwent renal CT angiography were evaluated. CT examination was performed with 100 mL of IV contrast material at an injection rate of 4mL/sec and a pitch of 6. In every case, arterial and venousphase volumetric data sets were acquired at 20 and 35 sec, respectively. Scans were reconstructed at 1.25 mm intervals for three-dimensional imaging using a MIP technique. Additional CT urographic images were obtained at 120 sec. Findings of CT angiography and orography were compared with surgical findings. Results CT angiography and surgical findings agreed in 96.6%(29 of 30 patients). In one patient CT angiography revealed three renal arteries in left, but surgery revealed one renal artery. Twenty- four patients underwent left nephrectomy, and six patients underwent right nephrectomy because supernumerary left renal arteries were detected on preoperative CT angiography. The length of left renal artery ranged from 18 to 110 mm (mean, 46.85mm), and that of right renal artery ranged from 26 to 105 mm(mean, 51.95mm). The length of left renal vein ranged 40-120(mean, 75.67mm) and that of right renal vein ranged 10-61 mm(mean, 35.17mm). CT angiography showed extrarenal pathology in 7 patients: atherosclerosis in abdominal aorta in one, pneumonia in one, gall bladder stone in one, liver hemangioma in two, and liver simple cyst in two patients, respectively. Conclusion CT angiography is very useful for preoperative planning in living kidney donors.
