장기요양 노인가족의 부양부담에 영향을 미치는 요인에 관한 연구

A Study on Factors Affecting the Caregiving Burden in Families of Long-term Care Elders - Focused on families using day care centers for the aged -

본 연구의 목적은 노인주간보호시설을 이용하는 가족부양자의 부양부담에 영향을 미치는 노인의 특성, 가족부양자 특성 그 리고 주간보호서비스 관련 특성들을 규명하는 것이다. 연구대상은 강원도 내 주간보호시설을 이용하는 60세 이상 노인의 가족부양자로서 주부양자인 125명이 최종 분석되었다. 분석방법은 다중회귀분석과 위계적 회귀분석을 실시하였으며, 주요 결과는 다음과 같다. 첫째. 노인의 인지기능 손상정도는 신체기능 손상정도보다 가족의 부양부담에 더 큰 영향을 미치는 요 인이었다. 둘째, 부양부담에 영향을 미치는 가족부양자 관련 요인으로는 건강상태와 1일 부양시간이 확인되었다. 건강상태 가 좋지 않고 1일 부양시간이 길수록 가족부양자의 부담은 증가하는 것으로 나타났다. 셋째, 서비스 이용 관련 특성으로는 직원의 친절도가 무엇보다 부양부담 감소에 중요한 요인으로 검증되었다. 넷째, 하위차원별로 가족의 부양부담에 상대적으 로 가장 큰 영향력을 갖는 요인은, 신체적 부담에 가족부양자의 건강상태가, 사회적 부담에 노인의 신체기능 손상정도가, 정서적 부담에 노인의 인지기능 손상정도가, 경제적 부담에 주간보호서비스 이용료 문제가 확인 되었다. 이들 연구결과들을 바탕으로 노인주간보호시설을 이용하는 가족부양자의 부양부담을 감소시킬 수 있는 보다 효과적인 실천방법과 정책개발을 논의하였다.

The purpose of this study is to identify factors related to elders, caregivers, and day care service, which affect the caregiving burden of families that use day care centers for the aged. The subjects of this study were the families of elders aged over 60 who were using day care centers for the aged, in particular, the main caregivers who assumed the primary responsibility to take care of the elders. Data were collected with the cooperation of all the 15 day care centers for the aged in Gangwon-do, distributing self-reporting structured questionnaires to elders’ families upon meeting, and collecting them upon farewell. A total of 125 questionnaires were used in the analysis. Collected data were analyzed using SPSS 12.0. The results of the analyses are as follows. First, caregivers’ burden was heavier when damage to elders’ physical/cognitive functions was severe. Second, caregiving burden was heavier when the caregiver’s health condition was poor and when the average caregiving time per day was long. Third, staff kindness was found to reduce the families’ caregiving burden. According to the results of hierarchical regression analysis, staff kindness appeared to reduce the influence of caregiving time per day. In conclusion, as practical and political suggestions for reducing the caregiving burden in families using day care centers for the aged, it is required to provide institutional support to lower the cost of service for low-income classes, to establish an integrated service system for caregivers rest through day care, short-term care, nighttime care, etc., to build a community-based care management system for enhancing psychological accessibility, to provide services such as bathing, rehabilitation and feeding for reducing the families physical burden, and lastly to develop various programs and practical interventions for families using day care services.
