신경인지재활치료가 편측무시환자의 시지각 기능과 상지기능에 미치는 효과

The Effect of Neurocognitive Rehabilitation on the Visual Perception and Ypper-limb Function of Neglect

본 연구의 목적은 편측무시를 보이는 좌측 편마비환자에게 신경인지재활치료를 접근하여 편측무시가 감소됨으로 시지각 기능의 향상에 따른 상지기능에 미치는 효과를 확인하고자 하여 향후 신경학적 손상 환자들의 치료적 질을 향상시키는데 기여하고자 한다. 방법: 본 연구의 대상자는 인천에 소재하는 B재활병원에 입원하고 있는 좌측 편마비 환자 중 편측무시를 진단 받은 자로 자연 회복 가능성을 최소화하기 위하여 발병 기간이 6개월 이상 경과하고 실험의 내용을 이해하고 적극적으로 참여 의사를 밝힌 자를 선정하였다. 본 연구의 참여자는 40세 남자로 2014년 1월 4일 집 앞에서 쓰러진 후 발생한 의식 변화로 대학병원으로 후송되어 자기공명영상촬영(Magnetic Resonance Image; MRI)에 의해 뇌내출혈과 경막하출혈으로 진단받은 환자이다. 대상자는 타병원에서 치료를 받다가 전문재활치료를 받기위하여 본원에 2014년 6월 5일 입원하여 2014년 6월 9일부터 2014년 8월 1일까지 8주 동안 주 5회신경인지재활치료를 실시하였다. 결과: 본 연구의 결과를 보면 상지기능은 뇌졸중상지기능검사에서 마비측 22점 향상, 퍼듀 페그보드 검사에서 마비측6개 향상, 그루브드 페그보드 검사와 젭슨-테일러 손 기능검사 마비측 수행불가능에서 수행 가능해졌다. 장악력 10㎏, 편측집기 6.5㎏, 손바닥 집기 5.5㎏, 손끝집기검사 3.5㎏으로 각각 증진 되었다. MVPT-3는 원점수가 17개 향상되었다. 결론: 신경인지재활치료가 편측무시의 감소에 효과가 있었고, 편측무시가 감소됨으로 시지각 기능이 향상되어 상지기능이 향상되는 효과를 나타내었다. 본 연구는 자연회복이 되지 않는 편측무시 환자들의 치료에 활용될 수 있을 것이고 향후 신경학적 손상 환자들의 치료적 질을 향상시키는데 기여할 수 있을 것으로 생각된다.

The object of this study is confIrm the decline in neglect through the administration of neurocog-nitive rehabilitation, causing an increase in visual perception function, and having a flow-on effect on the upper-limb function. By studying the effect on neglect patients with Lt. hemiplegia. Conclusion: The administration of neurocognitive rehabilitation caused an overall reduction the effects of neglect. By the reduction of neglect, the patient``s visual perception function were enhanced, leading to an overall improvement in their upper-limb motor function. Overall, this contribution will be useful in improving the quality of future treatment for patients with neurological damage. Methods: The subject for this study has been chosen from a group of patients that are admitted in B Rehabilitation hospital, located in Incheon, Korea. Out of left-side hemiplegia with neglect have suffered the condition for more than 6 months were selected. The participant for this study is a 40 year old male. The subject was admitted to the current location on the 5th of June 2014 in order to receive professional rehabilitation. The patient went through 5 instances of neurocognitive rehabilitation treatment over the period of 8 weeks, lasting from the 9th of June 2014, to the 1st of August 2014. Results: Upon looking at the results of this study, it showed an increase of 22 points in the Manual Function Test for upper-limb function, an increase by 6 pegs in the Purdue Pegboard test, and in the Grooved Pegboard Test and the Jebsen- Taylor Hand Function Test, the patient went from being unable to complete, to being able to complete the test. An increase of grip strength to lOkg, lateral pinch to 6.5kg, palm pinch to 5.5kg, tip pinch test to 3.5kg was shown in the patient.
