목적: 현재까지 국내에서 보고된 바 없는 안와 림프절을 처음으로 경험하여 이를 보고하고자 한다. 증례요약: 기저질환이 없는 24세 여자 환자가 2개월 전부터 발생한 좌측 위눈꺼풀 가쪽의 종괴를 주소로 내원하였다. 이학적 검사상 둥글고 경계가 분명한 피하 종괴가 유동적으로 촉지되었으며 통증, 부종 및 피부 변화는 보이지 않았다. 종괴의 양상과 위치 및 영상학적 검사상 유피종이 의심되었으며, 정확한 진단 및 치료를 위해 절제 생검을 시행하였다. 1.0 × 0.7 cm 크기의 종괴는 갈색의 둥근 형태였으며, 경계가 분명하였으나 뒤쪽으로는 골막과의 유착 소견이 관찰되었다. 조직병리학적 검사 결과 림프절염이 동반된 림프절로 진단되었다. 수술 후 재발이나 새로운 병변은 관찰되지 않았으며 환자도 이상증상을 호소하지 않았다. 결론: 안와에서 완전한 형태를 갖춘 림프조직은 아직까지 발견되지 않았으며 림프절 또한 존재하지 않는 것으로 알려져 있다. 하지만 본 증례를 통해 안와 종괴가 관찰될 경우 림프절의 가능성도 고려해야 하며 조직병리검사를 시행하여 확진해야 함을 알게 되었다.
Purpose: We report our experience with a case of orbital lymph node which has not been previously reported in the Republic of Korea. Case summary: A 24-year-old female patient with no underlying disease visited our hospital with a 2-month history of a mass at the lateral side of the left upper eyelid. On physical examination, a round, well-defined subcutaneous mass was palpable, and pain, swelling or a skin change were not reported or seen. The appearance, location, and imaging findings of the mass were sus-pected to be dermoid, and excisional biopsy was performed for accurate diagnosis and treatment. The 1.0 × 0.7 cm-sized, round-shaped and brown-colored mass was excised. The mass was well-defined but in the posterior part; it was attached to the periosteum. The mass was finally diagnosed as a lymph node with lymphadenitis by histopathological examination. No re-currence or new lesion was observed after surgery, and the patient did not complain of abnormal symptoms. Conclusions: A Complete orbital lymphoid tissue has not yet been found in the orbit, and lymph nodes are known to be absent. However, as in this case, when an orbital mass is observed, the possibility of lymph node should be considered and histopatho-logical examination should be performed to confirm it.
