우울증 환자의 건강관련 삶의 질에 영향을 미치는 요인

Factors Influencing Health related Quality of Life in Patients with Major Depression

본 연구는 우울증 환자의 건강관련 삶의 질에 영향을 미치는 요인을 파악하고자 제6기 국민건강영양조사를 이용하여 이차자료 분석을 수행하였다. 국민건강영양조사는 국민의 건강수준 및 건강행태를 파악하는 대표적인 국가 조사로, 제6기 국민건강영양조사는 2013년에서 2015년까지 전국적으로 시행되었다. 본 연구의 대상은 원 자료 전체 7,380명 중 의사로부터 우울증을 진단받고 현재 우울증이 있는 것으로 조사된 우울증 환자 전수 151명이다. 우울증 환자의 건강관련 삶의 질에 영향을 미치는 요인 파악을 위해 SPSS/WIN 23.0 및 STATA 13.0 프로그램을 이용하여 χ2-test, one-way ANOVA, multiple regression을 실시하였으며, 자료 분석은 2017년 12월부터 3개월간 수행하였다. 그 결과 대상자의 건강관련 삶의 질에 대한 회귀모형은 유의하게 나타났으며(F=9.469, p<.001), 전체 모형의 설명력은 37.8%였다 (Adjusted R2=.338). 우울증 환자의 건강관련 삶의 질은 주관적 건강상태가 좋다고 판단할수록(β =0.223, p=.003), 활동제한이 없을수록(β=0.300, p <.001), 최근 1년 동안 병의원에 가고 싶을 때 병의원에 내원 가능할수록( β=0.159, p=.029) 높게 나타났다. 우울증 환자의 건강관련 삶의 질에 영향을 미치는 요인들을 확인한 본 연구결과를 통해 우울증 환자에 대한 차별화된 건강교육 프로그램 개발의 필요성과 개입의 필요성을 확인하였다.

The purpose of this study was to examine factors influencing health-related quality of life using EQ-5D in patients with major depression. This study carried out seondary analysis using the data from the 6th Korean National Health and Nutrition Examination Survey. The subject samples of this study who were selected are 151 patients with major depression. The data were analyzed by using descriptive statistics, χ2-test, one-way ANOVA, multiple regression by the SPSS 23.0 and STATA 13.0 program. The mean score of EQ-5D was 0.81 in patients with major depression. The results of multiple regression analysis showed that subjective health status(β=0.223, p=.003), restriction of activity(β=0.300, p<.001), and available of hospital visitation(β=0.159, p=.029) had only significant effects on health-related quality of life(F=9.469, p<.001) in patients with major depression, explaining 37.8% of the variance(Adjusted R2=.338). Patients with major depression in Korea have low health-related quality of life compared to other subjects. In addition, factors influencing health-related quality of life differed from previous studies. Therefore differentiated nursing interventions are needed for patients with major depression to improve each of the factors affecting the patient’s health-related quality of life. It is meaningful that this study provided basic data for preparing a practical health education program to improve health-related quality of life in depressed patients.
