간호학생을 위한 임상상황기반 의사소통시뮬레이션 모듈개발

Development of Clinical Situation-based Communication Simulation Modules for Nursing Students

본 연구는 본 연구는 시뮬레이션을 처음 접하는 4학년 1학기 간호학생을 대상으로 임상상황에 기초한 의사소통 시뮬레이션 모듈을 개발하고 실행타당도 검증을 통하여 간호학생들의 의사소통 역량을 향상시킬 수 있는 시뮬레이션학습 전략을 개발 하기 위한 기초자료를 제공하고자 시도되었다. 이를 위해 2016년 1월부터 6월까지 간호사와 당뇨환자 간 의사소통에 중점을 둔 간호교육 내용이 포함된 상황을 시뮬레이션 모듈로 개발하였고, 실행타당도를 검증하기 위해 시뮬레이션 수업 경험이 없 는 일개 대학 4학년 1학기에 재학 중인 간호학생을 대상으로 표준화환자 활용그룹 25명과 단순마네킹 활용그룹 27명에 적용하여 모듈의 타당도를 검증하였다. 표준화 환자 활용그룹에서는 모듈 적용 과정에서 작성한 성찰일지의 내용을 추가적으로 분 석하였다. 연구결과, 단순마네킹 활용그룹의 사전·사후 의사소통능력과 의사소통 자기효능감은 사전·사후 유의한 차이가 없는 것으로 나타났다. 표준화환자 활용그룹에서 사전·사후 의사소통능력과 의사소통 자기효능감은 통계적으로 유의하게 향상된것으로 나타났다. 또한 성찰일지 분석결과 표준화환자를 활용한 실습 경험이 간호학생 자신의 의사소통기술을 성찰해보고 환자와 의사소통하는데 자신감을 가지게 하였으며, 의사소통의 중요성을 인식하는데 효과적이었음이 확인되었다.

The purpose of this study was to develop communication simulation module which can be utilized to enhance the communication skills of the nursing students and also verified the practicability of the developed module. Through the focus group interview with the clinical practitioner, we derived the frequent clinical situations between nurses, nurses and doctors, nurses and patients. Among them, in order to verify the practicability of simulation practice education targeting nursing students, we constructed the module containing nursing education that is focused on the communication between nurses and patients. We identified the communication skills and communication self-efficacy of nursing students before and after the application of the simulation module. According to the verification result of the practicability of the simulation module, no significant difference in pre and post communication skills and the pre and post self-efficacy of the simple mannequin utilization group was found. In the case of the standardized patient utilization group, the improvement of the pre and post communication skills and the communication self-efficacy was found to be statistically significant. In addition, based on the analysis result of the self-reflection diary, the practical experiences utilizing the standardized patients affected the confidence of nursing students in reflecting their communication skills and communicating with patients. Also, the experiences were reported to be effective for them to recognize the importance of the communication.
