60세 이상 고령환자군에서 핀치 피부절제법을 병용한 결막접근 아래눈꺼풀성형술의 효과

Effect of Transconjunctival Approach Lower Blepharoplasty Combined with Pinch Skin Excision Technique in Elderly Patients

목적: 핀치 피부절제법(Pinch skin excision technique)을 병용한 결막접근 아래눈꺼풀성형술이 피부이완 소견이 동반된 60세 이상의 고령환자에서도 효과가 있는지 알아보고자 하였다. 대상과 방법: 2013년 3월부터 2018년 2월 사이에 핀치 피부절제법을 병용한 결막접근 아래눈꺼풀성형술을 시행받은 후 6개월 이상경과 관찰한 60세 이상의 환자 39명을 대상으로 하였다(상안검성형술을 동반한 경우는 20명). 남자 16명, 여자 23명을 대상으로 수술성공률, 합병증의 발생 여부, 재발, 환자 만족도를 확인하였다. 결과: 39명 중 평균 6.3개월의 추적 관찰 기간 동안 수술 성공률은 97%였다. 재발은 없었고, 합병증은 5명이었으며 이 중 3명은 눈꺼풀겉말림, 2명은 결막부종이 있었는데, 눈꺼풀겉말림이 발생한 3명의 환자 중 1명은 가쪽눈꺼풀판띠고정술을 시행하였으며 결막부종을 호소한 2명은 경과 관찰 후 자연 호전되었고 환자 만족도는 4.26점이었다. 결론: 핀치 피부절제법을 병용한 결막접근 아래눈꺼풀성형술은 60세 이상의 고령의 환자에서도 높은 수술 성공률과 수술 만족도를 보였다. 이에 피부이완이 있는 환자에서도 미용적으로 만족스럽고 수술 후 합병증이 적어 피부접근법을 이용한 아래눈꺼풀성형술대신 시행되어도 효과적일 것으로 생각된다. <대한안과학회지 2019;60(3):205-209>

Purpose: The purpose of this study was to investigate the effect of transconjunctival approach lower blepharoplasty combined with pinch skin excision technique in elderly patients over 60-years-old. Methods: The study included 39 patients (16 male patients, 23 female patients) over 60 years of age and over a six-month of follow-up period who had undergone transconjunctival approach lower blepharoplasty combined with pinch skin excision technique. All included patient’s medical record were reviewed for success rate of operation, recurrence rate, postoperative complication occurrence and the degree of patients’ satisfaction. Results: The success rate of surgery during the mean follow-up period of 6.3 months was 97% among total of 39 patients. There were five cases of reported complications; 3 patients with ectropion in which 1 of 3 patients underwent tarsal strip operation for the correction of ectropion, 2 with chemosis. All patients were satisfied with the final results; Average satisfaction score of 4.26. Conclusions: The transconjunctival approach lower blepharoplasty combined with pinch skin excision technique in elderly patients had high success rates and patient satisfaction scores. Elderly patients with prominent lower eyelid fat with skin excess will satisfy with this technique for low complication rates and high cosmetic satisfaction. J Korean Ophthalmol Soc 2019;60(3):205-209
