The Mediated Effect of Depression on the Correlation with Degree of Dementia, Motor Performance Ability, and Quality of Life of the Elderly with Dementia

본 연구는 치매노인의 치매의 정도, 운동수행능력, 우울, 삶의 질을 평가하여 그 관계를 알아보고, 우울이 변수들에 미치는 매개효과를 알아보기 위함이다. 101명의 치매노인을 대상으로 치매의 정도, 운동수행능력, 우울, 삶의 질을 평가하였으며, 치매의 정도를 평가하기 위해 전반적 퇴행척도를 이용하였다. 운동수행능력을 평가하기 위해 운동평가척도를 사용하였으며, 우울은 한국판 단축형 노인우울척도, 그리고 삶의 질은 치매노인 삶의 질척도를 이용하여 평가하였다. 그 결과 유병기간(5년이하, 6년 이상)에 따른 평균비교에서 치매의 정도, 운동수행능력, 우울, 그리고 삶의 질에 대하여 유의한 차이를 나타내었다(p<0.05). 주요변수들의 상관관계에서 운동수행능력은 치매의 정도, 우울과 유의한 부적상관성이 있었고(p<0.05), 삶의 질과 유의한 정적 상관성을 보였다(p<0.05). 치매의 정도는 우울과 유의한 정적 상관성을 보였고(p<0.05), 삶의 질과는 강한 부적상관관계가 있었다(p<0.05). 우울은 삶의 질과 유의한 부적상관관계가 있었다(p<0.05). 변수들의 삶의 질에 미치는 영향과 우울의 매개효과에서 운동수행능력은 치매의 정도, 우울과의 관계에서 유의한 부적영향을 미쳤고(p<0.05), 삶의 질에는 유의한 정적 영향을 미쳤다(p<0.05). 본 연구는 치매노인과 치매의 정도, 운동수행능력, 우울 및 삶의 질에 유의한 관계가 있음을 확인하였으며, 치매의 장도와 운동수행능력이 삶의 질에 미치는 영향에서 우울의 부분매개효과를 입증하였다. 본 연구의 결과를 토대로 치매노인을 대상으로 임상에서 적용 가능하도록 적절한 가이드라인을 제시하고자 한다.

This study evaluates the degree of dementia, motor performance ability, depression, and quality of life of elderly with dementia. The subjects included 101 elderly with dementia. To measure motor performance ability, cognition, depression and quality of life. Among the major variables, Motor Assessment Scale (MAS) showed significant negative correlation with Global Deterioration Scale (GDS) and, Korean Short form of Geriatric Depression Scale (KGDS-SF), and positive correlation with Geriatric Quality of Life-Dementia (GQoL-D). GDS showed significant correlation with KGDS-SF (p<0.05), and negative correlation with GQoL-D. KGDS-SF only showed significant correlation with GQoL-D. Regarding the variables effects on the GQoL-D and mediated effect on KGDS-SF, MAS showed correlation with GDS and KGDS-SF, and positive effect on GQoL-D. A significant correlation between dementia and motor performance ability, cognition, depression, and quality of life can be observed, proving the partial mediated effect of depression with motor performance ability and cognition on quality of life. The based on the results of this study, appropriate guidelines are intended to be provided for clinical applications for the elderly with dementia.
