Comparative Dose Evaluation based on Various Scout Images and AEC Function in Multiple Scan exams: Neck-Chest Phantom Study

목적: 현재 암 환자들의 추적 검사 및 대부분 CT 검사를 한 번에 여러 부위를 진행하는 경우가 많다. 하지만 각 병원마다 scout image 획득 방식이 다르며, 이때 Automatic Exposure Control(AEC)가 어떻게 적용되는 지에 대한 연구가 부족하다. 대상 및 방법: 본 연구는 두께가 갑자기 달라지는 흉부와 경부의 multi-sacn 검사 시 scout image의 획득 방식에 따라 경부나 흉부에 최적의 선량을 부여하기 위하여 연구를 진행하였다. AEC 방식이 다른 두 제조사의 장비를 사용했으며, 경부와 흉부 multi-scan 검사를 재현하기 위해 인체 모형의 Rando 팬텀을 이용하였고 팬텀에 팔이 없기 때문에 소뼈를 사용하여 위팔뼈를 재현하였다. 결과: Multi-scan 검사 시 G사의 경우 마지막 scout image를 짧은 길이로 lat scout을 획득하였을 때 너무 적은 선량이 들어가기 때문에 문제를 발생시킬 수 있으며, 마지막 scout image가 영향을 미치기 때문에 마지막을 AP로 획득하는 것이 가장 이상적이라는 것을 확인할 수 있었다. 반면에 S사의 경우는 scout image 획득의 순서가 선량에 영향을 미치지 않으며, 경부의 scout image의 길이를 길게 획득 하였을 경우 가장 최적의 선량이 되는 것을 알 수 있었다. 결론: CT 검사를 실시하기 전에 정확한 scout image의 획득이 중요하다.

Purpose: Most of the patients who underwent follow-up examination and many CT examinations are done in many body part at one time. However, The scout image acquisition method differs from hospital to hospital, and research on how to apply Automatic Exposure Control(AEC) and to acquire ideal scouts for multi-scan exams is lacking. The purpose of this study was to investigate the best of scout image acquisition on multi-scan CT examination of chest and neck with various condition changes. Material and Method: We used the equipment of two different AEC systems and the Rando phantom of the human body model to reproduce the neck and chest multi-scan CT. To make arms on the Rando phantom, The cow bones were utilized. In the case of a multi-scan, The GE CT scanner can cause problems if the final scout image has a short length and is the lateral scout. Results: As a result, to use lateral scout image is prohibited. Finally, It was confirmed that obtaining the image by AP scout is ideal. On the other hand, in the case of the Siemens CT Scanner, the order of acquisition of the scout image has no effect on dose. Conclusions: It was found that the optimal dose was obtained when the length of the scout image of the neck was long. It is important to obtain accurate scout images before performing this CT scan.
