노인의료복지시설 거주노인 입소과정의 자기결정권과 생활요인에 관한 혼합 연구

A Study of Self-determination of Dwelling Elders in Nursing Home on Their Admission / Discharge Process

본 연구는 노인의료복지시설을 이용하는 노인의 입·퇴소 시 자기결정권이 어떻게 보장되고 있는지를 노인의 권리 관점에서 파악하고, 장기요양서비스의 제도개선 방안에 관한 실천적·정책적 함의를 제안하는 것을 목적으로 한다. 이를 위하여 1) 시설거주 노인대상 설문조사 2) 노인의료복지시설 종사자 대상 Focus Group Interview(FGI)를 실시하였으며 그 결과는 다음과 같다. 첫째, 시설의 입·퇴소 과정에서 자기결정권이 보장된 노인의 경우 그렇지 않은 노인에 비하여 우울 및 삶의 만족이 유의미하게 더 높은 것으로 나타났다. 둘째, 노인의료복지시설 종사자 대상 FGI결과, 시설 내 노인의 자기결정권 보장의 어려움, 노인의료복지시설에 대한 부정적 시선과 개선을 위한 노력, 제도적 변화의 필요성을 제안하였다. 또한, 노인의료복지시설의 부정적 이미지를 해소하기 위한 방안으로 ‘재활’을 바탕으로 한 지역사회복귀를 위한 기능의 회복을 돕는 기관으로서의 역할 전환을 확인하였다. 이를 바탕으로 노인의료복지시설에 거주하는 노인의 더 나은 삶을 위한 정책적·실천적 발전방향을 제언하였다.

The purpose of this study is to examine the self-determination of older persons in nursing home facility on the viewpoint of their rights, and to propose practical and policy implications for the improvement of long-term care services. To achieve this 1) quantitative survey for nursing home residents 2) Focus group interview with nursing home workers. were conducted, and the study result follows. First, the elderly who was gauranteed self-determination right in the process of admission and discharge showed significantly higher depression and life satisfaction than those who did not. Second, the FGI results showed that the difficulty of guaranteeing the self-determination of the elderly in the facility, the negative view and improvement of the elderly medical welfare facilities, and the necessity of change of the facilities were proposed. In addition, in a way of resolving the negative image of nursing home facilities, we suggested the change the role of facility from ‘wrapping up’ to ‘rehabilitation’. On the basis of results, the policy and practical implications for the better life of the elderly living in the nursing home facilities were suggested.
