생물학적 동등성을 증명하기 위한 추가시험의 통계적 분석

Statistical Analysis for Add-on Test in Bioequivalence Trials

제제 간의 생물학적 동등성을 증명하는데 실패했을 때 추가시험으로 얻은 자료를 추가하여 동등성을 증명할 수 있도록 하는 시험기준이 식품의약품안전처에서 시행되고 있다. 이 제도는 개인에 따른 제제의 생체이용률 차이가 예상보다 큰 고변동약물의 경우처럼 본시험에서 불충분한 표본크기 때문에 동등성을 보이는데 실패하였다고 판단되는 경우, 추가시험을 통하여 피험자수를 증가시켜 동등성을 증명할 수 있도록 하는 제도이다. 식품의약품안전처의 의약품동등성시험기준에는 추가시험을 통해 얻은 자료가 본시험의 자료와 일관성이 있음을 두 시험의 잔차제곱평균의 비율이 큰 차이가 없으며 시험 효과와 제제 효과와의 교호작용이 없다는 두 가지 사실로 증명하고 나서, 두 시험의 자료를 통합한 자료로 제제 간의 생물학적 동등성을 입증하도록 하고 있다. 현재 생동성시험에서 널리 사용되는 국내 개발 프로그램들은 본시험자료를 입력하여 간단히 생동성여부를 알아보고 식약처에 제출하는 생동성시험 결과보고서에 제시될 통계분석 결과를 얻는 용도로 사용되고 있지만 추가시험자료를 분석하는 기능을 없다. 본 연구에서는 본시험과 추가시험 자료를 통합분석하여 생동성여부를 알아보고 결과보고서에 사용될 통계분석 결과를 얻는데 수월하게 사용될 수 있는 SAS 프로그램을 제시하고, 이 때 사용되는 통계적 모형과 분석절차를 단계별로 간략히 설명하였다. 또한, 본 논문에서 주로 다룬 2×2 교차시험과 추가시험 이외에 생물학적 동등성 시험을 위한 다양한 설계와 통계적 모형에 대해 알아보았다.

The bioequivalence guideline of Korea allows an add-on test when a initial trial fails to show the bioequivalence between test and reference drugs. The sponsor may add more subject using an add-on study, when the first trial is considered to be failed because of an insufficient number of sample size for a highly variable drug, for example. The ministry of food and drug safety allows the bioequivalence to be proved using add-on study in two step procedure. After the consistency between initial and add-on studies is shown by the two facts that the ratio between the mean square errors of two studies are not big and the interaction between drug and study effects are not significant, the bioequivalence can be shown using pooled data of initial and add-on study. Software programs currently popular in Korea use initial trial data to produce analysis result for the report to be submitted to the ministry of food and drug safety. But, they cannot analyze add-on study data. We present a SAS program which can be easily used to analyze initial and add-on study data to produce analysis result for the report. The statistical model and the analysis procedure to show the bioequivalence through add-on study is explained precisely with an illustrative example.
