청년 흡연, 어떻게 접근할 것인가?

Young adults and smoking: A qualitative exploration

본 연구는 청년 흡연의 중요성을 부각하고, 생애주기 특성을 바탕으로 한 담배규제정책 개발에 기초자료를 제시하고자 한국 청년의 흡연 행태를 사회문화적 맥락을 중심으로 탐색하는 것을 그 목적으로 한다. 이를 위하여 2017년 질병관리본부‘청년 흡연 행태 심층조사’의 질적 자료를 사용하였다. 이는 만 19~29세 청년 63명을 대상으로 흡연 행태(매일 흡연자, 간헐적 흡연자, 과거 흡연자)와 성별(남성, 여성)에 따라 총 8개의 초점집단토의를 통해 수집된 자료이며, 주제 분석을 사용하여 본 연구의 목적에 따라 결과를 재분석하였다. 연구 결과 도출된 다섯 가지 주제 중 ‘청년 흡연의 단계’는 청년의 흡연 행동의 변화와 변화 가능성의 특성을 보여준다. 그 외 네 가지 주제인 ‘관계에 따른 흡연양상’, ‘흡연을 촉진하는 기회’, ‘성별에 따른 차이와 차별’, ‘담배규제정책에 대한 회의적인 시각’은 상호작용하며 청년 흡연 행태와 인식에 영향을 주는 상황적 맥락이다. 또한, 이들은 ‘간헐적 흡연’, ‘본격적 흡연’, ‘잠재적 흡연’의 각 단계에 머무르게 하거나, 단계 간의 전환 요인으로 작용하여 ‘청년 흡연의 단계’에 영향을 준다. 청년의 흡연 예방 및 금연을 위해서는 이러한 생애주기 특성을 고려한 개인, 개인 간, 조직 수준의 정책적 개입이 요구되며, 이는 청년의 당사자성을 바탕으로 도출되어야 한다.

This study was to explore and describe the smoking of Korean young adults in a life-course perspective. Thematic analysis was performed on the focus group data collected with 63 participants in 2017. Among five overarching themes, the ‘stages of youth smoking’, which consisted of a cycle of ‘intermittent smoking’, ‘regular smoking’ and ‘potential smoking’, illustrated the multi-phased, multifaceted characteristics of smoking in young adulthood. Four other themes included smoking patterns according to relationships , ‘opportunities to promote smoking , gender differences and discrimination and skeptical view on tobacco control policy , which describe the contextual background and interaction factors of the smoking behaviors of Korean young adults. Policy interventions at the individual, interpersonal and organizational levels that take into account life-course characteristics are required along with the current tobacco control policies promoted at the national level.
