상급종합병원 간호사의 환자안전문화 인식과 환자안전관리 활동의 관계에서 조직몰입의 효과

Effect of Nurses Organizational Commitment in Relationship between Perception on Safety Culture and Patient Safety Management Activities in Tertiary Hospitals

본 연구는 간호사가 지각하는 환자안전문화 인식과 환자안전관리 활동의 관계에서 조직몰입의 조절효과와 매개효과를 규명하기 위하여 시도된 서술적 조사연구이다. 연구대상은 2016년 1월 1일부터 1월 30일까지 B광역시 소재 상급종합병원인 I 대학교 부속병원에 근무하는 간호사 중 편의추출하여 연구의 목적을 이해하고 연구참여에 동의한 연구대상자중 선정기준에 만족한 150명을 대상으로 하였다. 자료수집은 구조화된 자가보고식 설문지를 이용하였으며, 연구도구는 일반적 특성, 환자안전문화 인식, 환자안전관리 활동, 조직몰입으로 구성되었다. 자료분석방법은 SPSS 21.0 프로그램을 이용하여 기술통계, 피어슨 상관관계분석, 위계적 회귀분석으로 분석하였다. 환자안전문화인식은 환자안전관리 활동(r=.330), 조직몰입(r=.407)과 양의 상관관계를 나타냈다. 그리고 환자안전관리 활동은 조직몰입(r=.274)과 양의 상관관계를 나타냈다. 간호사의 환자안전문화 인식과 환자안전관리 활동관계에서 조직몰입은 부분매개역할을 하였으며, 조절효과는 없었다. 결과적으로 환자안전문화 인식이 증가할수록 조직몰입이 증가하고, 간접적으로 환자안전관리 활동에 영향을 준다는 것을 의미한다. 즉 환자안전관리 활동의 향상을 위해 조직몰입에 관심을 가지면서 환자안전문화 인식을 증가시키도록 병원내 환경 조성이 필요하겠다.

The purpose of this study was to determine the mediator or moderator role of nurse’ organizational commitment in the relationship between perception on safety culture and patient safety management activities. A cross-sectional design was adopted. The sample included 150 nurses working in three tertiary university hospitals in B city by means of structured questionnaires. Data was analyzed with descriptive statistics, Pearson’s correlation coefficient and hierarchial multiple regression using the SPSS 21.0 program. Organizational commitment showed partial mediating effects between perception on safety culture and patient safety management activities. However, organizational commitment did not show moderating effects. Organizational commitment, perception on safety culture and patient safety management activities had a strong relationship. This findings suggest the importance of nurses’s organizational commitment to increase patient safety management activities. It is important for managers to create an organizational climate that promotes the organizational commitment. Future studies are encouraged to replicate the findings and also to better understand effect of organizational commitment in the relationship between perception on safety culture and patient safety management activities.
