간접흡연 노출 시 담배를 꺼달라는 주장행위 영향 요인

Factors Associated with Assertive Behavior to Secondhand Smoke in Patients with Asthma

본 연구는 천식 환자의 간접흡연 노출 시 담배를 꺼달라는 주장행위 정도 및 영향요인을 파악하기 위한 서술적 조사연구이다. 연구대상자는 경기도 소재 1, 2차 의료기관의 외래 치료를 받는 천식 환자 중 지난 한 달 동안 1주일에 1회 이상 간접흡연에 노출된 경험이 있는 환자 140명이다. 천식 환자의 간접흡연 노출 시 담배를 꺼달라는 주장행위의 관련요인은 ASE 모델(attitudes-social influence-efficacy model)의 세 가지 요인인 태도(담배를 꺼달라는 주장행위를 당연하다고 생각하는가), 사회적 영향(주장행위를 하는 타인을 본 경험이 있는가) 및 자기효능(흡연자에게 담배를 꺼달라고 말할 자신감이 있는가)과 더불어 간접흡연의 괴로움, 간접흡연의 위험성 지각, 담배를 꺼달라고 요구하여 담배를 꺼준 성공적인 주장행위 경험을 조사하였다. 자료 분석은 SPSS 24.0을 이용하여 서술통계와 t-test/ANOVA, 다중회귀분석을 하였다. 천식환자의 간접흡연 노출 시 담배를 꺼달라는 주장행위 정도는 평균 2.19점(1-5점 범위)으로 39.3%의 대상자가 주장행위를 전혀 하지 않았고 6.4%만이 매우 자주 주장행위를 하였다. 천식환자의 주장행위는 자기효능이 높을수록, 성공적인 주장행위 경험이 많을수록, 타인의 주장행위를 많이 인지할수록, 그리고 간접흡연의 괴로움이 클수록 많았다. 이 네 가지 요인들은 주장행위를 38.1% 설명하였다.

The aim of this study was to identify assertive behavior level and factors related to assertive behavior when exposed to secondhand smokes in patients with asthma. The study was conducted to apply attitudes-social influence-efficacy (ASE) model. The three main factors of the ASE model were attitudes towards assertive behaviors, social influence, i.e., perceived assertive behavior to others, and self-efficacy of assertive behavior. In addition, annoyance with secondhand smoke, perceived health risk of secondhand smoke, and successful experience of assertive behavior were included. We conducted a cross-sectional study with 140 patients with asthma. The mean level of assertive behavior (range 1-5 scores) was 2.19. Among participants, 37.6% was not at all assertive, while 6.3% was very often assertive. Higher assertive behavior showed significant correlation with a higher level of self-efficacy, successful experience of assertive behavior, perceived assertive behavior to others, and annoyance with secondhand smoke. These factors was responsible for the assertive behavior by 38.1% in patients with asthma.
