노인의 자아존중감이 건강관련 삶의 질에 미치는 영향

The Influence of Self Esteem on the Elder s Health Related Quality of Life: Mediating Effect of Health Empowerment

본 연구는 노인의 자아존중감과 건강관련 삶의 질의 관계에서 건강 임파워먼트의 매개효과를 확인하고자 시도된 서술적 횡단적 조사연구이다. 이를 위하여 D광역시에 위치한 노인복지관 및 사설 노인대학, 충남지역의 노인회관에 다니는 65세 이상의 노인 151명을 대상으로, 2015년 10월 1일부터 2015년 11월 30일까지 직접 면접조사의 방법으로 자료를 수집하였다. 수집된 자료 중 부적절한 응답인 12부를 제외한 139부를 통계분석에 활용하였다. 수집된 자료는 IBM SPSS 20.0 프로그램을 이용하여 기술통계 방법, Pearson의 상관계수, 위계적 회귀분석 및 Sobel test를 이용하여 분석하였다. 그 결과 대상자의 평균 연령은 74.5세였으며, 평균 질병개수는 1.91개로 나타났다. 자아존중감은 평균 3.62점(1∼5점), 건강 임파워먼트는 평균 3.49점(1∼5점)을 보였다. 건강관련 삶의 질은 신체적 영역점수와 정신적 영역점수는 각각 평균 50.48점, 53.50점으로 나타났다. 노인의 자아존중감과 건강관련 삶의 질의 관계에서 건강 임파워먼트의 매개효과 여부를 검증한 결과 건강 임파워먼트는 통계적으로 유의미한 매개효과를 보였다. 이상의 연구 결과, 노인의 건강관련 삶의 질을 높이기 위해서는 노인의 자아존중감과 함께 건강 임파워먼트를 개선하기 위한 중재가 함께 제공되어야 할 것이다.

The purpose of this study was to investigate the mediating effects of health empowerment (HE) on the correlation between self esteem (SE) and the elder s health related quality of life (HQOL). This study was conducted as a descriptive cross-sectional survey with 139 elder people who attend at a educational institution for elder people, located in D city, S province. The data was collected from October 1st, 2015, to November, 30th, 2015 with structured questionnaire. The average of age of research subjects was 74.5 years and the average number of disease was 1.91. With Sobel test, mediating effect of HE on the correlation between SE and HQOL was significant (Z=3.95, p<.001). The hierarchical regression model with SE and HE and other demographic and health related variables against HQOL was statistically significant (F=13.10, p<.001). And this model could explain approximately 41% of HQOL of elder people. The findings of this study imply that a broad approach including HE and SE of elder people should be considered to improve the HQOL for elder people.
