간호업무성과의 예측인자로서의 임상적 의사결정능력과 자기효능감

Canonical Correlation among Clinical Decision Making, Self-efficacy and Nursing Performance

본 연구는 간호사들의 임상적 의사결정능력과 자기효능감, 간호업무성과간의 상관성을 조사하기 위해서 수행되었다. 연구대상자는 일 대학의 RN-BSN 과정생 93명이었으며, 자가보고식 설문을 통해 임상적 의사결정능력, 자기효능감, 간호업무성과를 측정하였다. 자료분석은 서술적 통계, 다중회귀와 정준상관을 이용하였다. 본 연구 결과, 임상적 의사결정능력과 간호업무성과 간에는 한 개의 정준상관이 확인되었으며 임상적 의사결정능력과 자기효능감 간에는 두 개의 정준상관이 존재하였다. 첫 번째 변량은 일반적 자기효능감과 사회적 자기효능감이 높은 것은 독자적, 비독자적, 대인관계적 간호업무를 성공적으로 수행하는 것과 정적인 상관성이 존재함을 보여주고, 두 번째 변량은 사회적 효능감이 높은 것과 독자적 간호업무를 잘 수행하지 못하며 대인관계적 간호업무를 잘 수행하는 것과 상관성이 존재함을 보여주었다. 임상적 의사결정능력과 간호업무성과 간의 관련성에 있어 자기효능감의 매개효과가 나타났으며 총 설명변량은 =.32(F=43.14, p<.001)이었다. 따라서 자기효능감은 임상적 의사결정능력과 간호업무성과간의 매개효과를 갖는 것으로 볼 수 있었고, 간호업무성과를 설명하는 데는 두 가지의 경로(임상적 의사결정->자기효능감->간호업무성과 vs. 임상적 의사결정->간호업무성과)가 있음을 알 수 있었다. 따라서 간호사의 임상적 의사결정능력 및 자기효능감을 증진시킬 수 있는 임상적응 프로그램 개발을 제언한다.

The purpose of this cross-sectional study was to specify the relationship among clinical decision making, self-efficacy and nursing performance. The participants of this study were 93 nursing students in RN-BSN course work. Self administered questionnaires that included a clinical decision making, self-efficacy and nursing performance were used to collect data in one university in Korea. Descriptive statistics, multiple regression, canonical correlation were used for data analysis. Clinical decision making was related to nursing performance with single significant canonical variables while self-efficacy was related to nursing performance with two significant canonical variables. The first variate showed the higher two subcategories of self-efficacy showed the better nursing performance. The second variate indicated that nurses felt higher social self-efficacy showed lower independent nursing performance and higher interpersonal relationship. Path analysis showed that self-efficacy mediated the relationship between clinical decision making and self efficacy. The selection of variables for identifying the effect on nursing performance should be considered the direction of relationship among clinical decision making, self-efficacy and nursing performance.
