암환자의 연령에 따른 증상심각성, 증상관련 지장성, 우울, 투병의지 및 사회적 지지와의 관계 비교연구

The Difference in Symptom Severity, Symptom Interference, Depression, Fighting Spirit, and Social Support between Younger and Older Patients with Cancer

본 연구는 암환자들의 증상심각성, 증상관련 지장성, 우울, 투병의지 및 사회적 지지 정도가 연령(65세 미만, 65세 이상)에 따라 차이점이 있는지 분석하고 두 연령군의 증상심각성에 미치는 영향요인을 파악하기 위한 서술적 조사연구이다. 본 연구는 J시에 위치한 암센터에서 치료를 받고 있는 암환자 270명을 분석에 이용하였다. 연구결과 증상심각성 차이로는 노인 암환자에서 건망증을 더 많이 호소하는 것으로 나타났으나(p=0.003), 두 군 간의 증상심각성 차이는 보이지 않았다(p=0.961). 반면에, 노인 암환자에서 증상으로 인하여 일반적인 활동(p=0.045), 일(p=0.014), 기분(p=0.045), 보행(p=0.001)에서 성인 암환자보다 지장을 많이 받는 것으로 나타났다. 우울은 노인 암환자에서 더 높았고(p<0.001), 투병의지는 더 낮은 것으로 나타났다(p=0.009). 한편 사회적 지지 정도는 두 군 간의 차이를 보이지 않았다(p=0.210). 증상심각성에 영향을 미치는 요인으로는 65세 미만 성인 암환자에서는 우울 정도가 높을수록(p<0.001), 혼자 사는 경우(p=0.018)에, 65세 이상의 노인 암환자에서는 투병의지가 낮을수록(p=0.001), 폐암 환자일수록(p=0.003), 우울 정도가 높을수록(p=0.025) 증상의 심각성 정도가 높은 것으로 확인되었다. 본 연구는 의료진들이 성인 암환자와 노인 암환자가 호소하는 증상경험과 우울 및 투병의지를 이해하는데 도움을 줄 것이며 각각의 연령군에 적합한 증상관리를 위한 프로그램을 개발하는데 필요한 기초자료로 활용될 수 있을 것이다.

The purpose of this study was to evaluate the differences in symptom severity, symptom interference, depression, fighting spirit, and social support between younger (<65 years) and older (≥65 years) cancer patients, and also to identify the predictive factors to influence symptom severity in each age group. A cross-sectional descriptive design was used. A total of 270 patients completed the questionnaire (157 younger and 113 older patients). Few age-related differences were found except difficulty remembering in symptom severity (p=0.003). Older patients reported more higher symptom interference with general activity (p=0.045), work (p=0.014), mood (p=0.045), and walking (p=0.001). Older patients had more depression (p<0.001) and lower fighting spirit (p=0.009) than younger patients. Social support was no difference in each age group (p=0.210). In younger patients, higher depression (p<0.001) and living alone (p=0.018) explained higher symptom severity. In older patients, lower fighting spirit (p=0.001), lung cancer (p=0.003) and higher depression (p=0.025) explained higher symptom severity. This study help nurse understand the age-related difference in symptom experience and provide information for developing appropriate nursing strategies to manage multiple symptoms in patients with cancer based on age group.
