노인의 성별에 따른 자살생각과 영향요인

Suicidal Ideation and Affecting Factors according to Sex in Elderly by using Korea Health Panel Data 2010

본 연구는 노인의 성별에 따른 자살생각의 현황을 확인하고, 노인의 성별에 따라 자살생각에 미치는 영향 요인을 규명하기 위해 시도되었다. 본 연구는 한국보건사회연구원과 국민건강보험공단이 전국규모로 수행한 한국의료패널 2010년도 연간 통합 데이터를 활용하였고, SPSS WIN version 20.0을 이용하여 카이제곱(chi-square) 검정과 다중로지스틱 회귀분석(multiple logistic regression)을 실시하였다. 연구대상은 자살생각 경험 여부에 대한 질문에 응답한 65세 이상 노인의 자살생각 경험은 남성은 7.3%, 여성은 12.6%로 나타났다. 노인의 자살생각에 미치는 요인은 남성노인의 경우 우울감이 있는 경우가 없는 경우보다 8.94배(CI 4.27-18.69), 가족 갈등 스트레스가 있는 경우가 없는 경우보다 1.96배(CI 1.00-3.85), 주관적 건강상태가 ‘좋음’보다 ‘나쁨’에서 2.74배(CI 1.09-6.88), 규칙적인 식사를 하는 경우보다 못하는 경우에서 2.68배(CI 1.24-5.78)로 나타났다. 여성노인의 경우 배우자가 있는 경우보다 없는 경우보다 1.70배(CI 1.12-2.59), 우울감이 있는 경우가 없는 경우보다 5.98배(CI 3.80-9.43), 경제적 스트레스가 있는 경우가 없는 경우보다 1.83배(CI 1.04-3.21), 가족 갈등 스트레스가 있는 경우가 없는 경우보다 1.84배(CI 1.20-2.82)로 나타났다. 따라서 노인자살예방을 위하여 노인의 성별에 따른 영향요인을 고려한 자살예방 프로그램이 필요하다.

The purpose of this study is to measure suicidal ideation and to investigate the affecting factors according to sex in elderly by using Korea health panel data 2010. The data was analyzed by chi-square and multiple logistic regression using SPSS WIN 20.0 program. The major finding of this study were that the ratio of suicidal ideation was 7.5% in male and 12.6% in female. Affecting factors to suicidal ideation in male elderly revealed that presence of depression and family conflict stress, poor self-rated health status, irregular diet had more suicidal ideation by 8.94 times (CI 4.27-18.69), 1.96 times (CI 1.00-3.85), 2.74 times (CI 1.09-6.88), 2.68 times (CI 1.24-5.78). Affecting factors to suicidal ideation in female elderly revealed that no spouse, presence of depression, economic stress and family conflict stress had more suicidal ideation by 1.70 times (CI 1.12-2.59), 5.98 times (CI 3.80-9.43), 1.83 times (CI 1.04-3.21), 1.84 times (CI 1.20-2.82). respectively. This study is meaningful as an attempt to measure suicidal ideation and to analysis affecting factors suicidal ideation according to sex in elderly. A careful and thorough examination may be needed for further study.
