Rasch 모형을 이용한 노인장기요양보험 대상자의 기능상태 평가 연구

A Study on Rasch Model for the Assessment of Person’s Functional Conditions in Long-Term Care Insurance System

우리나라는 급속한 인구고령화에 따라 간병수발이 필요한 고령자가 증가하게 되어 해당 가족 및 사회적인 부담을 주고 있다. 노인장기요양보험은 간병수발의 부담을 덜어주고자 정부가 2008년부터 본격적으로 시행한 이후 2011년 12월 현재 38만여명의 노인들이 그 혜택을 보고 있다. 노인장기요양보험의 시행에 중요한 요소는 수급대상자의 등급판정에 있으며, 등급판정을 위한 데이터는 대상자들에 대한 신체 특성 및 기능을 측정한 값이 사용된다. 본 연구에서는 기능상태 영역별 평가항목 데이터에 대해 국민들이 이해하기 쉽도록 Rasch 모형을 이용하여 100점 만점으로 환산한 점수를 제시하고자 한다. Rasch 모형은 WINSTEPS를 이용하여 분석하는 것이 일반적이나 본 연구에서는 로지스틱회귀모형을 이용하였다. Rasch 모형이 로지스틱회귀모형과 수리적으로 대응됨을 보이고 노인장기요양보험의 자료에 대한 로지스틱회귀분석의 결과와 WINSTEPS의 결과가 동일함을 확인하였다.

Long-term care insurance is social insurance system that provides benefits to the elderly who have difficulty taking care of themselves for a period of at least 6 months. It have been started in July, 2008. In this study, we focus on the assessment of person’s functional conditions using the data of measures from service time survey 2011. Item response theory models are widely used in educational and psychological testing. Usually the item response theory models are estimated using the special computer programs, such as, WINSTEPS. Rasch model one of item response theories can be applied to develop the index of functional conditions which vary from 0 to 100 scores. It can be showed that there exists a one-to-one correspondence between the parameters of a special logistic regression and the Rasch models using the service time survey.
