노인요양시설의 급식 및 영양 관리 체계 구축을 위한 연구

영국사례를 중심으로

본 연구는 영국 케어홈의 급식 및 영양에 관한 법적제도적 차원에서의 관리체계를 살펴봄으로써 한국의 노인복지시설 급식 및 영양관리체계 구축을 위한 시사점을 도출하는데 목적이 있다. 한국의 노인복지시설 급식관리 체계에 대해 검토한 여러 연구들은 노인의 급식의 양과 질을 국가가 보장할 수 있는 법적 제도적 방안이 시급히 마련되어야 한다고 주장함에 따라 한국에 앞서 노인복지시설의 급식 및 영양에 관한 국가차원의 법적 제도적 방안을 마련한 영국의 사례를 문헌연구에 기반해 살펴보았다. 그 결과, 영국의 노인복지시설 급식 및 영양관리는 2000년의 ｢케어기준법｣과 2008년의 ｢보건 및 사회적 케어법｣ 그리고 이들 법에 따른 후속 조치들에 기반하고 있음을 확인할 수 있었다. 구체적으로 ｢케어기준법｣에서는 국가케어기준위원회와 국가최소기준에 관한 법적 근거를 마련하였으며 후속 조치들을 통해 급식 영양 및 영양관리에 관한 국가최소기준과 서비스 제공자의 준수사항을 마련하고 있었다. 나아가 급식 및 영양문제를 직접적으로 담당하는 지방정부는 중앙정부의 법적 조치에 따라 실정에 맞는 구체적인 시행지침을 마련하여 운용하고 있었다. 이러한 영국의 조치들을 기반으로 우리나라의 노인복지시설 급식서비스의 질을 높이기 위해, 법적체계 구축 및 이에 따른 서비스 표준지침 마련; 영양실조 선별도구(MUST)를 사용한 노인복지시설 입소자 전체를 대상으로 한 영양상태 파악; 영국의 케어품질위원회(CQC)와 같은 기구 설립을 통한 노인복지시설 급식 및 영양의 질적 보장; 그리고 급식 및 영양 서비스 제공에 있어 핵심 역할을 하는 영양사의 충분한 확보 등을 시사점으로 제언하였다.

This study explores the implications of establishing a meal security system for the elderly welfare facilities in Korea by examining the legal and institutional management system for food and nutrition in care homes in the United Kingdom. Several studies on meals and nutrition management for the elderly welfare facilities in Korea conclude that the country urgently requires legal and institutional measures to guarantee the quantity and quality of meals served to the elderly. Based on literature reviews and focusing on the roles of the central and local governments, we conducted a case study on the United Kingdom, which has established national-level legal and institutional measures for food service and nutrition in elderly care homes. Food and nutrition management for elderly care homes in the United Kingdom is based on the Care Standards Act 2000, the Health and Social Care Act 2008, and follow-up measures under these laws. The Care Standards Act 2000 established the legal basis for the National Care Standards Commission and the national minimum standards. Through follow-up measures, these national minimum standards for food and nutrition management and the provisions regulating care home providers were established. Furthermore, the local governments, which are directly responsible for food and nutrition regulation, have established and implemented specific implementation guidelines in accordance with the legal provisions of the central government. Based on these measures in the United Kingdom, this study establishes the legal system as an implication for Korea and presents standard service guidelines accordingly. It proposes assessing the nutritional status of all residents of elderly welfare facilities using the Malnutrition Universal Screening Tool(MUST) and ensuring quality assurance of meals and nutrition through the establishment of an organization similar to the B ritish Care Quality Commission (CQC). It a lso proposes adeq uate recruitment of dietitians, who play a key role in providing meals and nutrition services.
