전·후기 노인 암 환자의 재원일수 결정요인

The Determinant of the Length of Stay of Young-Old and Old-Old Patients Hospitalized Owing to Cancer

본 연구는 암으로 입원한 전체 노인을 연령대별로 나누어 전기 노인(65~74세) 및 후기 노인(75세 이상)의 일반적 특성 차이, 재원일수 차이 및 재원일수 결정 요인을 파악하고자 시행되었다. 질병관리청의 퇴원손상심층조사 원시자료를 활용하였으며, 복합표본 교차분석과 복합표본 일반선형모형(t-검정, 분산분석, 회귀분석)을 사용하여 분석하였다. 분석결과, 일반적 특성은 전기노인은 수술시행과 수도권 병원이 높은 반면, 후기 노인은 의료급여, 응급실 경유가 높게 나타났다. 일반적 특성에 따른 재원일수는 전·후기 노인 공통으로 여성, 의료급여, 응급실 경유, 암 질병 동반, 수술시행, 100~299병상일 경우가 더 길었고. 후기노인의 평균재원일수가 12.4일로 전기노인의 11.7일보다 0.7일 더 길었다. 재원일수에 주요하게 영향을 미치는 요인은 성별을 제외하고 전·후기 노인 모두에서 진료비지불방법, 입원경로, 부진단, 주 수술여부, 병원소재지, 병상규모 등으로 밝혀졌다. 이러한 결과는 입원 노인 암환자의 재원일수를 줄이기 위해 후기 노인에서는 여성인 경우와 전·후기 노인 모두에서는 입원경로가 응급실 경유나 외래경유인 경우, 부진단이 암 질병인 경우, 병원 소재지가 경상도인 경우, 의료기관의 병상 규모가 100~299병상, 300~499병상, 500~999병상인 경우에 재원일수 관리를 위한 교육 및 적극적인 방안이 모색되어야 함을 시사한다.

This study aimed to understand the differences of general characteristics of young-old (age 65-74) and old-old (over age 75), length of hospital stay, and the determinants of hospital stay based on data of the Korean National Hospital Discharge In-Depth Injury Surveys provided by Korea Disease Control and Prevention Agency (KDCA). A complex sample cross tabulation analysis (chi-square test) and complex sample generalized linear model (t-test, analysis of variance, regression analysis) were used to analyze the data. As a result, primary factors affecting the length of hospital stay were drawn to medical fee payment method, admission route, comorbidity, operation, location and bed size of hospital in young-old and old-old except gender, Conclusively, to reduce the length of hospital stay, female of old-old, admission via emergency room (ER) and out patient department(OPD), cancer as comorbidity, Gyeongsang-do as location of hospital, and 100-299, 300-499, 500-999 bedsize of hospital for both case of young-old and old-old should be managed proactively.
