골수이형성증후군 환자에서 발생한 안와염증, 시신경유두부종과 삼출성 망막박리

Orbital Inflammation, Papilledema, and Exudative Retinal Detachment in Myelodysplastic Syndrome

목적: 골수이형성증후군으로 진단받고 외래진료 중이던 환자에게서 발생하여 급속하게 진행한 안와염증, 시신경 유두부종과 삼출성 망막박리의 증례를 보고하고자 한다. 대상과 방법: 63세 남자 환자가 내원 1일 전부터 발생한 좌안 통증 및 안구 돌출을 주소로 내원하였다. 내원 당시 나안시력은 우안 1.0, 좌안 0.6이었고 좌안의 안검부종 및 결막하출혈을 동반한 결막부종, 안구돌출이 관찰되어 입원치료를 시작하였다. 안와 컴퓨터단층촬영 검사상 좌측 안검부종, 안구돌출 및 구후부에 염증소견이 관찰되었다. 결과: 안검부종 및 결막부종은 호전되는 양상을 보였으나 입원 7일째부터 좌안시력이 저하되기 시작하였고 안저검사상 시신경유두부종, 삼출성 망막박리가 관찰되었으며 입원 8일째 급격한 시력저하와 함께 좌측 중대뇌동맥협착과 좌측 대뇌경색이 발생하여 의식저하된 상태로 지내다 입원 16일째 사망하였다.

Purpose: To report a case of orbital inflammation, papilledema, and exudative retinal detachment in myelodysplastic syndrome patient. Methods: A 63-year-age man with myelodysplastic syndrome visited our clinic due to proptosis and ocular pain in the left eye. Uncorrected visual acuity of his left eye was 0.6. Lid edema, chemosis, and subconjunctival hemorrhage were revealed in the left eye. Orbit computerizd tomography scan showed lid swelling, proptosis and inflammation of retrobulbar area in the left eye. After admission, instillation of levofloxacin and fluorometholone acetate 0.1% 4 times a day with systemic antibiotics was started. Results: Although lid edema, chemosis and subconjunctival hemorrhage were resolved, visual acuity of left eye was decreased at 7th day of admission. Papilledema, exudative retinal detachment were noted. Magnetic resonance imaging (MRI) scan images of brain were obtained because of worsened mental status. MRI scan showed left cerebral infarction with left middle cerebral artery stenosis. At 16th day of admission, the patient expired in spite of medical and ophthalmological treatment.
