굴절수술대상 근시 환자에서 동공중심과 시축의 위치적 상관관계

The Positional Relationship between the Pupil Center and the Visual Axis of Myopia Patients

목적: 굴절수술을 목적으로 내원한 근시환자에서 동공중심과 시축의 위치적 상관관계를 조사하고자 하였다. 대상과 방법: 굴절수술을 목적으로 내원한 근시환자 50명, 100안을 대상으로 NIDEK OPD-SCAN(Nidek, Gamagori, JAPAN), ORBscan IIz (Bausch&Lomb, USA) 기기를 이용하여 광순응 동공중심(photopic pupil center)에서 시축(visual axis)까지 거리와 방향, 동공크기, 굴절력, 각막크기, 전방 깊이를 조사하였고, 광순응 동공중심에서 시축까지 거리(PDist)와 다른 요소들과의 연관성이 있는지도 조사하였다. 결과: 근시환자 100안에서 광순응 동공중심에서 시축까지의 평균값은 0.198±0.107 mm이며, 광순응 동공크기는 3.788±0.648 mm 그리고 중등도조명시각 동공크기(mesopic pupil diameter)는 6.353±0.741 mm였다. 그리고 평균 굴절력은 -5.524±2.321 D (구면렌즈대응치, SE)이었다. 광순응 동공중심에서 시축까지의 거리(PDist)는 구면렌즈대응치(SE)와 유의한 상관관계를 보였으며, 구면렌즈대응치가 증가할수록 광순응 동공중심에서 시축까지의 거리가 증가하였다(p＜0.01). 결론: 근시가 심한 환자일수록 동공중심과 시축의 거리의 차이가 커지므로 굴절수술 시 이를 고려해야 할 것이다.

Purpose: To evaluate the positional relationship between the pupil s center and the visual axis in myopia patients as a substitute for a refractive surgery. Methods: We used NIDEK OPD-SCAN (Nidek, Gamagori, JAPAN) and an ORBscan IIz (Bausch&Lomb, USA) instrument to examine both of the eyes of 50 myopia patients and research various factors including the distance and direction from the photopic pupil center to the visual axis, pupil diameter, refractive power, size of the cornea, anterior chamber depth, and the correlation between the distance from the photopic pupil center to the the visual axis (PDist). Results: The average distance from the photopic pupil center to the visual axis was found to be 0.198± 0.107 mm, the photopic pupil diameter was 3.788±0.648 mm, and the mesopic pupil diameter was 6.353± 0.741 mm. The distance from the photopic pupil center to the visual axis showed a meaningful correlation with a spherical equivalent, and as the spherical equivalent increased, the distance from the photopic pupil center to the visual axis increased (p＜0.01). Conclusions: We suggest that, as the severity of a patient s myopia increases, the distance from the photopic pupil center to the visual axis increases. We believe this should be taken into considerations prior to refractive surgery.
