안와 이물의 일차 제거술 후 잔류된 안와 및 상악동 이물

A Case of Retained Wooden Sino-orbital Foreign Body

목적: 우안부 관통상을 입고 안와 이물의 일차 제거술 후 잔류된 안와 및 상악동내 나무 이물을 성공적으로 제거한 경험을 보고하고자 한다. 대상과 방법: 산에서 자전거를 타고 가다가 나뭇가지에 우안부를 찔려 우안 공막 열상, 우상안검 열상 일차 봉합술 및 안와이물 제거술 시행 후에도 지속적인 부종, 안구운동시 심한 통증, 안구운동장애를 보인 33세 남자 환자에서 안와하벽골절 정복술 시행시 잔류 안와이물을 발견하였다. 결과: 전신마취하에 안와골절 정복술 위해 안와 하벽 접근 시 나무 조각으로 보이는 이물이 안와 하벽 골절부위와 상악동에 걸쳐 있었고 주변으로 염증반응이 관찰되었다. 하결막낭을 통해 이물을 성공적으로 제거하였고, 항생제 희석액으로 관주하면서 남아 있는 작은 조각들을 제거하였다. 조직검사결과 육아조직을 확인할 수 있었으며, 술 후 3개월째 통증, 부종이 감소하였고, 안구운동의 경한 호전을 보였다. 결론: 잔류된 안와내 이물은 주변조직에 영향을 미쳐 안구 운동장애, 복시, 사시, 안와 봉와직염, 안와농양 등 심각한 합병증을 유발할 수 있으며, 특히 나무이물은 영상학적 진단 및 이물질의 완전한 제거가 어려워 안와 관통상의 수술 후 간과되기 쉽다. 따라서 일차 수술 후에도 지속되는 심한 염증반응에 대해서 잔류 이물의 가능성을 고려해야 한다.

Purpose: To report an experience of retained wooden foreign bodies after primary surgery of penetrating eye injury. Methods: We found retained wooden intraorbital foreign bodies at the orbital inferior wall reconstruction surgery of a 33-year-old man complaining of persistent periorbital swelling, ocular pain and restricted eyeball movement after primary operation of ocular penetrating injury. Preoperative CT scan showed inferior orbital wall fracture and hypodense area in maxillary sinus. Results: In approaching to inferior wall, hard wooden mass and surrounding inflammatory tissue were found at the fractured site and maxillary sinus. The mass was grasped and withdrawn through inferior fornix. The residual conjunctival chemosis and pain subsequently resolved and limitation of movement of the right eye improved postoperatively. Conclusions: Retained orbital foreign bodies can cause serious complications including extraocular muscle restriction, diplopia, strabismus, orbital cellulitis, and abscess. Wood has a density similar to air and fat and can be difficult to distinguish from soft tissue in orbital computed tomography. Therefore, in cases where a inflammatory reaction sustained after primary surgery, the possibility of retained wooden orbital foreign body should be suspected.
