미숙아망막병증의 선별검사도중 우연히 발견된 양측성 후 구형수정체(Posterior Lentiglobus) 1예

Bilateral Posterior Lentiglobus Detected by Screening Examination of Retinopathy of Premature

목적: 미숙아망막병증 선별검사도중 우연히 발견된 양측성 구형수정체(Lentiglobus)를 경험하였기에 보고하고자 한다. 대상과 방법: 재태연령 27주 2일 0.83 kg으로 출생한 환아로 미숙아망막병증의 위험으로 30주 5일부터 안저검사를 시행하였다. 결과: 환아는 stage I의 미숙아망막병증이 있었으나 자연적으로 호전되었고 추적관찰 중 재태연령 39주부터 도상검안경을 통한 적색반사가 수정체의 중심부와 주변부가 다른 양상으로 관찰되었으며 점차 진행하여 42주부터 완벽한 기름방울이 떠있는 모양의 구형수정체가 양안에서 관찰되었다. 시축을 가리는 수정체 혼탁은 관찰되지 않아서 경과관찰 하기로 하였다. 결론: 구형수정체는 진행성의 수정체 발달이상으로 대부분 초기에 경한 양상으로 시작하여 늦은 소아기까지 발견되지 않는 경우도 흔하다. 3세에서 15세 사이에 동반된 백내장과 함께 진단되는 경우가 보통인데 본 증례와 같이 출생 초기 짧은 기간에 진행성의 전형적인 양안의 구형수정체가 발견되는 경우는 드물어 문헌 고찰과 함께 보고하는 바이다.

Purpose: To report a case of bilateral posterior lentiglobus in a premature infant by screening examination for retinopathy of prematurity. Methods: An 830 g 27 week 2 day gestation infant was consulted for screening examination about retinopathy of prematurity. Fundus examed 30 weeks 5 days gestation age. Results: Stage I retinopathy of prematurity improved spontaneously. Ring like reflex appeared in red reflex through indirect ophthalmoscope from 39 weeks gestational age in both eye and looks like oil droplet in water in the central red reflex at 42 weeks completely. Axial length was 17.74/17.65 mm in both, anterior chamber depth was 2.16/2.13 mm, lens diameter was 3.84/3.89 mm. There was no specific finding in B-scan. Central posterior lentiglobus portion had −0.25 Dsph −1.00 Dcyl * 90A refractive error in cycloplegic refraction. There was no opacity that cover visual axis. Conclusions: Posterior lentiglobus is progressive well circumscribed globular bulging of the posterior capsule of the lens. Generally it is detected with accompanied cataract between 3 and 15 year of age but this case shown bilateral progressive posterior lentiglobus in a short period at premature infant. So we report this rare case with literature review.
