We report a case of paranasal sinus mucocele manifested by diplopia, which was treated with endoscopic marsupialization. A 51-year-old female visited our clinic complaining of binocular diplopia for a month. Ocular examinations showed exophthalmos, adduction and elevation limitation on her right eye. Right anterior ethmoidal sinus mucocele was diagnosed by contrast-enhanced CT and endoscopic marsupialization was performed. On first post-operative day, diplopia and limitation of ocular movement disappeared. There was no postoperative complications and recurrence of the mucocele until 49 months after operation. In patients with acute diplopia, comprehensive investigation for underlying pathology is mandatory. The patient with mucocele has to be evaluated with radiologic study. After the confirmation of diagnosis, endoscopicmarsupialization is could be a successful strategy of treatment.
