자가 안구적출을 시도한 정신분열증 환자 1예

Attempting Autoenucleation in a 47-Year-Old Schizophrenia Patient

목적: 결박된 손으로 자가 안구적출을 시도한 정신분열증 환자 1예를 보고하고자 한다. 증례요약: 정신분열증으로 치료 중인 47세 남자환자가 스스로 안검 및 안구를 잡아 뜯어 응급실로 내원하였다. 우안의 비정상적인 동공반응과 상하 누소관 손상이 있었으며, 수술 중 360도 결막 열상과 내 ,외, 상, 하직근 파열이 관찰되었다. 우측 눈꺼풀, 누소관, 결막 그리고 안구 테논 조직의 일차봉합술을 시행하였고 수술 후 고농도 부신피질호르몬을 정맥 내로 투여하였다. 수술 후 1달째 시행한 섬광 시유발전위도 검사에서 우안의 반응이 없고, 안구운동의 제한과 상안검의 이완이 지속되었다. 결론: 정신질환이 있는 환자의 경우 자해의 일종으로 자가 안구 적출이 발생할 수 있기 때문에 주의를 요한다. 자해를 반복해서 시도하는 경우에는 수술 후에도 환자 상태를 면밀히 관찰하는 것이 필요하겠다.

Purpose: To report a case of a self-mutilation of his eyeball and eyelid. Case summary: A 47-year-old male with paranoid schizophrenia attempted to tear out his eyelid and eyeball by hand tied up with restraining band. He had already sustained self-inflicted eyelid laceration using a pen. Emergency surgical exploration was done under general anesthesia. There were avulsive injuries of upper and lower eyelid. Canalicular and 360-degree conjuntival lacerations were present. Four extraocular muscles were ruptured and retinal hemorrhage was noticed. Primary wound repair was performed. Conclusions: Autoenucleation can be one of the Self-harm of patient with Psychiatric disorder. Psychiatric referral for self-harm and close observation are mandatory even after operation, due to the possibility of repeat episodes and the increased risk of suicide.
