단순 열공망막박리에서 망막절개술 유무에 따른 결과 비교

Analysis of the Results after Vitrectomy with or without Retinotomy for Rhegmatogenous Retinal Detachment

Purpose: This study is designed to compare the outcomes of vitrectomy with retinotomy and vitrectomy without retinotomy investigated in primary vitrectomy for rhegmatogenous retinal detachment (RD). Methods: From March 2011 to March 2014, 80 eyes of 80 patients were retrospectively analyzed. Anatomic results, functional outcomes and complications were compared between eyes treated vitrectomy with retinotomy and eyes treated vitrectomy without retinotomy. Results: Among 80 eyes of 80 patients, the vitrectomy with retinotomy group had 51 eyes of 46 patients and vitrectomy without retinotomy group had 29 eyes of 29 patients. There was no significant difference in demographic and preoperative variable between two groups. Anatomic success after a single operation was obtained in 51 eyes (100%) of the vitrectomy with retinotomy group and 26 eyes (89.7%) of the vitrectomy without retinotomy group (p=0.099). Recurrence was significant statistically occurred in 3 eyes (10.3%) in the vitrectomy without retinotomy group. Visual acuity improved logMAR 0.3 or more in 25 eyes (49.0%) of the vitrectomy with retinotomy group and 12 eyes (41.4%) of the vitrectomy without retinotomy group (p=0.203). Conclusions: Retinotomy of rhegmatogenous retinal detachment affected significantly on the primary success rate for surgery. In conclusion, safe and appropriate retinotomy of rhegmatogenous retinal detachment will be great help on the success of operation.
