A Case of Lacrimal Gland Ductal Cyst Abscess

Purpose: We report the clinical characteristics and managemnet of a typical lacrimal ductal abscess case. Case summary: A 41-year-old woman presented with a 5-day history of chemosis, injection, erythema, and a tender mass on her left upper eyelid, especially the left superotemporal fornix. Based on computed tomography image showing a dilated lacrimal ductal system and abscess pocket, we diagnosed a lacrimal ductal abscess, which may be related with previous lacrimal ductal cyst. And then, she was prescribed antibiotic eyedrops every 2 hour and received intravenous antibiotics. After no response to 3 days of conservative treatment, surgical treatment involving incision, drainage, and marsupialization was performed. Two weeks later after the operation, the patient had good functional and anatomical status, was free from pain, discomfort. Conclusion: A lacrimal ductal cyst may progress to an abscess, and this should be suspected especially if the patient complains of a palpable tender mass, conjunctival swelling, and lacrimation. If an abscess is suspected, administration of intravenous antibiotics as well as surgical intervention such as marsupialization should be considered to prevent future complications.
