임신 중 재발된 시신경 유두 출혈 환자에서 보인 시신경 유두 드루젠 1예

A Case of Optic Disc Drusen Found with Recurrent Optic Disc Hemorrhage in Pregnancy

Purpose: We report a case of optic disc drusen associated with recurrent optic disc hemorrhage. Case summary: A 37 years-old woman was referred with recurrent optic disc hemorrhage. She was in pregnancy and took regular follow up, because of blurred vision and floater in her left eye after headache. She showed normal best corrected visual acuity, normal pupillary light reflex and normal color vision in both eyes. She showed enlarged blind spot in visual field test. The optic disc hemorrhage and blurred disc margin were found in superior temporal part of optic disc in her left eye. Fluorescein angiography showed block fluoroscence in superior temporal part of optic disc in left eye. The optical coherence tomography showed high reflectivity and heterogeneous lesion between retinal pigment epithelium and outer segment. There is no remarkable change in blood coagulability test. The magnetic resonance imaging showed no lesion. After 2 weeks, there was no optic disc hemorrhage. Conclusions: Optic disc drusen can be found incidentally without symptom. The hemodynamic change caused by pregnancy could make recurrent optic disc hemorrhage in patient with optic disc drusen. It could be helpful to consider the possibility of optic disc drusen to avoid unnecessary test and treatment, when patients have optic disc hemorrhage during pregnancy.
