인플루엔자백신 접종 후 발생한 급성후부다발판모양색소상피병증

Acute Posterior Multifocal Placoid Pigment Epitheliopathy following Influenza Vaccination

Purpose: To report a case of acute posterior multifocal placoid pigment epitheliopathy (APMPPE) following influenza vaccination. Case summary: A 62-year-old woman complained of unilateral blurry vision 10 to 14 days after influenza vaccination. At an initial visit, her right eye’s visual acuity and intraocular pressure was 0.5 and 24 mmHg respectively, and mild inflammatory cells were found in the anterior chamber and vitreous. Intraretinal fluid in the fovea, resembling cystic lesion, was also observed through spectral domain-optical coherence tomography (SD-OCT). Fluorescein angiography revealed unilateral multifocal early hypofluorescent lesion and late hyperfluorescent lesion. But indocyanine-green angiography revealed unilateral multifocal early and late hypofluorescent lesion. We diagnosed unilateral APMPPE and continued close observation. A one day later, anterior chamber cells disappeared and intraretinal fluid in the fovea was improved in SD-OCT, disappearing after one month. At six months follow-up, visual acuity became 1.0 with no recurrences. Conclusions: APMPPE is rare after human influenza vaccination. This is the first case, in Korea, of APMPPE after the influenza vaccination.
