포괄수가제의 전면 도입은 어떻게 가능하였는가?

How was the Introduction of DRGs (Diagnosis Related Groups) Possible?: A Political Management Perspective

포괄수가제는 환자에게 제공되는 의료서비스의 종류나 양에 관계없이 질병군 별로 미리 책정된 일정액을 지불하도록 하는 제도로, 기존 행위별수가제 하의 단점을 보완하기 위하여 도입이 논의되었다. 그런데 포괄수가제는 전면 도입과정에서 의료계의 강력한 반발로 인해 전면 도입까지 18년이라는 긴 시간이 소요되었다. 그렇다면 보건당국은 어떻게 의료계와의 첨예한 갈등 상황에서 포괄수가제의 전면 도입을 성공적으로 이끌어 낼 수 있었는가? 본 연구는 이 질문에 대해 보건당국이 실시한 전략을 Moore(1995)의 정치적 관리의 관점에서 분석하였다. Moore(1995)의 정치적 관리는 공공가치를 창출하기 위하여 정책을 도입하고자 하는 정책관리자가 자신의 권한 범위 밖의 이해관계자들로부터 정책에 대한 지지와 정당성을 확보하기 위한 전략적 기법을 의미한다. 특히, 본 연구는 보건당국이 포괄수가제 도입에 실패하였던 2003년과 성공을 거두었던 2011년에 각각 펼친 전략을 비교하여 분석하였다. 분석결과, 의료계의 반대를 약화하고 제도에 대한 폭넓은 지지를 확보하는 기업가적 옹호(entrepreneurial advocacy) 기법이 2003년의 실패를 딛고 2011년에 포괄수가 제의 전면 도입을 성공적으로 가져오는 데 크게 기여하였다. 주목할 점은 보건당국이 공공부문 마케팅과 협상 등의 다른 기법을 유기적으로 연계하여 우호적인 환경을 창출해 낸 결과로 기업가적 옹호 기법을 성공적으로 펼칠 수 있었다는 것이다. 이러한 분석을 바탕으로 본 연구는 포괄수가제와 같이 이해관계의 충돌로 제도의 도입에 난항이 예상될 때 제도를 도입하고자 하는 정책당국의 관리전략에 대해 실천적 함의를 제공한다는 점에서 의의가 있다.

A diagnosis-related group (DRG) is a patient classification system that standardizes prospective payment to hospitals and encourages cost containment initiatives. Health authority of South Korea proposed institutional change from Fee-for-service to DRG-based payment system with the purpose to enhance financial sustainability of national health insurance. However, it took 18 years to the implementation of DRGs, due to the harsh opposition of medical community. Then, how could health authority successfully introduce DRGs overcoming such an opposition? To answer this question, this study took Moore(1995)’s political management perspective as an analytical framework. With this framework, this study comparatively analyzes health authority’s strategies of two time span; 2003 when the authority failed to introduce DRGs and 2011 when the authority made success. The analysis shows that successful entrepreneurial advocacy was especially primary in introducing DRGs among five strategic techniques. One important finding is that health authority created a favorable environment by connectively applying other techniques like public sector marketing and association, so that application of entrepreneurial advocacy could have been successful. This study concludes by discussing the implications of these findings for successful policy management.
