일본 지역포괄지원센터를 중심으로 한 커뮤니티 케어(community care) 고찰

2008년 노인장기요양보험이 시행되고 10여 년 간의 시행착오를 거쳐서 노인장기요양보험제도가 안정화에 접어들었지만, 장기요양등급과 비등급의 분화 및 이용서비스의 단절화 등의 문제는 아직 풀어야 할 과제로 남아있다. 저출산현상과 함께 노인인구가 급증하게 되면서 노인장기요양보험의 재정적 과제 또한 사회문제이다. 2018년 보건복지부는 커뮤니티케어 로드맵을 발표하고 관련 선도사업을 추진해가는 것으로 한국노인복지의 큰 방향을 제시하였다. 따라서 본 연구는 2014년 의료개호일괄법과 지역포괄케어시스템을 선도적으로 시작한 일본의 경험을 고찰하고 특히 지역포괄센터의 실질적인 내용과 시스템을살펴봄으로써 한국의 커뮤니티케어에 함의를 찾고자 한다. 무엇보다 지역포괄센터가 제시하고 있는 의료·보건·복지의 연계방안 및 실질적인 사람중심의 케어(person-centered care)가 시사하는 바를 살펴보고자 한다.

Korea has introduced long-term care insurance in 2008 and now steps into the stable stage after about 10 years’ experiences. There are several tasks remain; classification of insurance beneficiaries and fragmentation of services by providers. Financial costs are one of the most important issues, as well. In 2018, starting leading businesses related to community, Korea indicated an innovative community care road map. This paper, therefore, examines Japanese experiences through medical-care combined act and community-based integrated system. Especially, exploring community general support center leads us to find a meaningful implication in community care system. Most of all, both network solution of medi-health-welfare links care and person-centered care are importantly discussed.
