연명의료 자기결정 보장을 위한 국내 의료기관윤리위원회 설치·운영 현황 및 관리 방안

The Establishment and Operation of Institutional Ethics Committees in South Korea and their Management Plan to Guarantee Patients’ Self-Determination over Life-Sustaining Treatments

연명의료결정 및 이행과 관련하여 임상현장의 다양한 갈등을 잘 해결하고 환자의 자기결정권을 최대한 존중하기 위해서는 의료기관윤리위원회의 역할이 중요하다. 본 연구는 의료기관윤리위원회 설치기관과 미설치기관을 대상으로 현재 운영 현황과 문제점 등을 파악하여 향후 제도의 올바른 정착을 위한 방안을 모색하고자 한다. 2019년 11월 4일부터 14일까지 전국 의료기관윤리위원회 설치 116개 기관, 미설치 305개 기관을 대상으로 온라인 설문조사를 시행하였다. 응답은 의료기관윤리위원회 간사 등 관련 종사자를 대상으로 1개소 1응답을 원칙으로 하였다. 조사결과 의료기관윤리위원회 운영에 따른 문제점으로는 신규 위원들을 대상으로 하는 교육의 어려움과 전담인력 부족으로 인한 업무의 효율성 저하, 재정적 지원의 필요성이 지적되었다. 의료기관윤리위원회 미설치기관의 경우 필요한 지원사항으로는 의료기관윤리위원회 업무에 대한 매뉴얼 및 효율적인 운영을 위한 인력 제공, 위원회 위원들을 대상으로 하는 연명의료결정법 관련 교육 지원이었다. 의료기관윤리위원회 설치 확대 및 연명의료 관련 자기결정권 강화를 위해, 향후 국내 실정에 부합하는 의료기관윤리위원회의 역할 및 관련 위원 역량, 교육 등을 포함한 질 관리 관련 추가 연구가 필요하겠다.

Institutional ethics committees play an important role in resolving various conflicts in clinical practices that are involved in deciding and implementing life-sustaining treatments and extending maximum respect to patients self-determination. This study explores a plan to help the proper establishment of the program by identifying the current operation of and issues with the institutional ethics committees at the organizations where they were established or where they were yet to be installed. An online questionnaire survey was used in this study with 116 participating organizations where the ethics committees were established and 305 organizations where they were yet to be installed. The data were collected between November 4 and 14, 2019. The survey was filled out once by the administrative secretary of each institutional ethics committee. The survey revealed issues with the operation of institutional ethics committees, such as difficulty in educating new committee members, lowered work efficiency due to insufficient staffing, and a lack of financial support. The needed support for the organizations where institutional ethics committees were yet to be installed was for the provision of a manual on the operations of the ethics committee and staff for its efficient operation and education on 「Act on hospice and palliative care and decisions on life-sustaining treatment for patients at the end of life」 for committee members. Installing more institutional ethics committees and strengthening the patients self-determination for life-sustaining treatments require further studies on the role of institutional ethics committees, the competencies of members, and qualitative management, including education.
