학교장면에서의 중도중복장애학생 건강관리 지원 방안 탐색

A Study on the Health Care Support Plan for Students with Severe Multiple Disabilities in School Scenes

연구목적: 학교장면에서의 중도중복장애학생의 건강관리 지원 과정을 살펴보고, 지원 형태를 유형화함으로써 효율적인 건강관리 지원 방안에 대해 알아보고자 하였다. 연구방법: 이 연구는 근거이론 분석을 통해 중도중복장애학생에게 건강관리 지원 과정에 미치는 요인들을 살펴보았다. 연구참여자는 특수교사 5명, 보건교사 2명, 학부모 1명, 의사 1명, 간호사 1명, 의료행정가 2명을 목적 표집 하였고, 1:1로 심층면담을 실시하였다. 연구결과: 면담자료를 분석한 결과 98개의 개념과 22개의 하위범주, 8개의 상위범주가 도출되었다. 중도중복장애학생의 건강관리 지원 과정의 중심현상은 법적·정책적 근거가 마련되어 있지 않음에도 불구하고, 특수교사, 보건교사, 학부모가 각자도생하고 있었으며, 그 과정에서 십시일반 하는 것으로 나타났다. 지원 형태는 보건교사와 특수교사는 대상학생의 주치의와 연계하여 건강관리 지원을 제공하는 ‘학교중심 지원’과 간호사가 학교에 상주하며 건강관리 지원을 전담하는 ‘전담간호사 지원’을 실시하고 있었다. 또 학교와 지역 내 의료기관이 업무협약을 체결하고 건강관리 지원을 실시하는 ‘학교·의료기관 협력 지원’을 실시하고 있었다. 결론: 이 연구는 중도중복장애학생의 건강관리 지원 과정에서 드러난 지원형태를 유형화함으로써 향후 효율적인 건강관리 지원 방안을 모색하였다는데 의의가 있다.

Purpose: The purpose of this study is to examine the process of health care for students with severe multiple disabilities(SMD) in school scenes and to investigate efficient health care. Method: This study conducted 1:1 interviews with five special education teachers, two health education teachers, one parent, one doctor, one nurse, and two medical staff who provide health care support for students with SMD, and conducted a grounded theory analysis. Results: As a result of the study, 98 concepts, 22 subcategories, and 8 upper categories were derived. Special teachers, health teachers, and parents were cooperating to support health care for students with SMD, and the types of support were categorized into school-centered support and school nurse support . Schools and medical institutions signed a business agreement and implemented ‘School-Hospital MOU’ to provide health care support. Conclusion: The study is meaningful in that it sought efficient medical service measures in the future by typifying the types of support revealed during the health care process for students with SMD.
