희귀·난치성질환자 가족의 가족적응에 영향을 주는 요인 분석

The Factors Affecting Family Adaptation in Families of patients with Rare Diseases

본 연구는 희귀·난치성질환 환자와 가족의 가족부담감, 가족탄력성, 자조모임 활동이 가족적응에 미치는 영향을 파악하는 데 목적이 있다. 이를 위하여 자조모임에 참여한 경험이 있는 환자 37명과 가족구성원 208명을 대상으로 수집한 자료를 분석하였다. 연구결과 첫째, 인구사회학적 특성 중 월수입, 질환 수용 정도, 양육/부양자가 지각하는 자신의 건강상태, 돌봄 기간, 치료제 유무, 의료비 부담, 환자의 장애 정도가 가족부담감, 가족탄력성, 가족적응에 영향을 주는 요인이었다. 자조모임 참여도와 만족도에 영향을 주는 요인은 양육/부양자가 지각하는 자신의 건강상태, 질환 수용 정도로 확인되었다. 둘째, 질환의 치료, 예후, 관리의 정도가 가족부담감, 가족탄력성, 가족적응, 자조모임 참여와 만족도에 영향이 있음을 확인할 수 있었다. 셋째, 희귀·난치성질환 가족부담감과 가족적응의 관계에서 가족탄력성의 매개효과는 유의하였다. 넷째, 희귀·난치성질환 환자와 가족의 가족부담감, 가족탄력성, 그리고 가족적응 간의 관계에서 자조모임의 조절효과는 유의하지 않았다. 마지막으로 본 연구결과에 대한 의의, 제한점 및 후속연구를 위한 제언을 제시하였다.

The aim of this study was to determine the effects of family burden, family resilience, and the activities of a patient support group on family adaptation in the patients with rare diseases and their families. The data were collected from 245 patients and their families who participated in patient support groups. The results of the study were summarized as follows: First, family burden, family resilience, and family adaptation were affected by monthly income, the disease acceptance level, perceived health status by caregivers, duration of illness, existence of treatment modality, the burden of medical expenses, and the disability level of patients among the socio-demographic variables. Satisfaction with participation in the patient support group affected perceived health status of caregivers and the disease acceptance level. Second, treatment modality, prognosis, and level of standard care had an impact on family burden, family resilience, family adaptation, and satisfaction with participation in patient support group. Third, family resilience played mediating effects between family burden and family adaptation in patients with rare diseases and their families. Fourthly, no significant moderating effect of patient support groups was found on family burden, family resilience, and family adaptation in patients with rare diseases and their families. Finally, the significance and limitations of the study was discussed along with future research.
