Acute renal infarction causes irreversible ischemic damage with varied misleading manifestations resulting in diagnostic delay, misdiagnosis, and treatment leading to renal damage. A 66-year-old man with atrial fibrillation presented with simultaneous aphasia and bilateral flank pain. Brain magnetic resonance imaging showed multiple left-sided acute cerebral infarcts, suggesting embolic causes. And then, we performed abdominal computed tomography to know the cause of persistent bilateral flank pain. As a result, the patient was diagnosed with acute bilateral renal infarction. Clinicians should be aware of the possibility of concomitant cerebral and renal infarction, especially if the patient has acute flank pain with neurological signs or symptoms.
