The Concept of Health as Ability and Its Application to Medical Justice Theory

코로나바이러스감염증-19와 관련하여 큰 문제로 제기되고 있는 것이 의료자원 분배이다. 중환자실 입원 순서, 백신 공급을 넘어 사회 전반에서 누구에게 먼저 의료자원을 제공할 것인지에 관한 쟁점이 확대되고 있다. 이런 논의는 의료정의론의 범위인데, 이를 적절히 전개하기 위해선 건강 개념에 관한 접근이 확립되어야 한다. 1980년대 이후 제기된 건강 개념은 이전 건강을 상태로 설정하던 것에서 벗어나 건강을 능력으로 정의하고 그 함의를 다루었다. 대표적으로 노르덴펠트의 전체론적 건강 개념, 후버의 적응과 자기관리 능력이 여기에 속한다. 이런 건강 개념은 이전의 개념이 건강을 이상화하거나 생물학적 접근에 국한하는 것을 벗어나 다양한 영역으로 건강 논의를 확장하는 특징을 지니고 있다. 먼저 두 건강 개념을 정리하고, 이런 건강 개념 변화가 의료정의론 논의에서 어떻게 나오는지 살폈다. 의료정의론은 차별과 의료자원 분배 논의를 다루며, 특히 후자에서 건강 개념은 그 체계를 구성하는 핵심이나 주목받지 못했다. 의료자원 분배에 관한 대니얼스와 시갈의 논의에서 건강 개념이 어떻게 차이가 나는지를 알아보고 이를 최근의 건강 개념과 연결한 다음, 코로나바이러스감염증-19 상황에서 벌어지는 의료자원 분배 논의에 이를 적용할 수 있는지를 살펴 보았다. 이 논의는 최근의 건강 개념 변화를 설명하고, 의철학적 접근이 의료정의론 담론을 확장할 수 있음을 보이기 위함이다.

COVID-19 raises the issue of medical resource allocation as a major social problem. Beyond the order of admission to intensive care units and the allocation of vaccines, issues are expanding over who to provide medical resources first in society. This discussion is within the scope of the medical justice theory. For the detailed discussion, however, the concept of health that shapes justice theory should be properly established. Since the 1980s, the concept of health has been changed from health as a state to health as an ability. Nordenfeld’s holistic health, Huber’s health as the ability to adapt and self-manage are examples of the recent concept of health. This concept has characteristics of expanding discussions about health into a variety of fields, compared to previous concepts that idealize health or limit it in biological dialogues. First, we organized the two health concepts and investigated how these changes in health concepts influence discussions on medical justice theories. The theory of medical justice deals with the discussion of discrimination and the distribution of medical resources, and its purpose is bringing health to individuals and societies. However, the concept of health was neither the core nor the focus of the system. In Daniels’ and Segall’s discussions on the distribution of medical resources, we examined how the concept of health differed and linked it to the latest health concept. Based on this, we investigated whether it could be applied to the discussion of medical resource distribution in the COVID-19 pandemic. This discussion intends to explain recent changes in health concepts and to show that an approach from the philosophy of medicine can extend the discourse on medical justice.
