일본 의철학ㆍ의료윤리 연구의 현황과 과제

Trends and Prospects of Studies on the Philosophy of Medicine and Medical Ethics in Japan: Focusing on the Activities and Studies of the Japanese Association for Philosophical and Ethical Researches in Medicine

본 논문에서는 일본의 의철학ㆍ의료윤리 연구의 역사를 간략하게 개관하고, 일본의학철학ㆍ윤리학회의 활동과 연구를 통해 최근 10년간의 연구 동향을 살펴보았다. 그 결과, 일본 의철학ㆍ의료윤리 연구의 특징은 첫째, 생명공학과 첨단의료의 발달로 인간강화, 재생의료, 정밀의료, 신경윤리를 다루는 연구가 등장했다는 것이다. 둘째, 학회의 창립 취지에 부응하게 의과대학의 철학과 의료윤리교육에 대한 관심과 연구가 계속되었다. 셋째, 고전적인 주제로서 인간, 의학의 본질, 그리고 낙태, 안락사, 호스피스 등 생명의 시작 및 종말과 관련된 연구가 이어졌으며, 온정주의보다 개인의 결정이 점점 중시되면서 자기결정권, 자율성, 사전동의를 다루는 연구들이 활발해졌다. 넷째, 일본에서 일어난 사건, 동일본 지진이라는 재해, 초고령사회라는 점 등 일본의 특수한 사건과 상황이 연구에 반영되었다. 그러나 일본의 연구는 대체로 영미권의 이론과 논의에 의존하고 있어서 일본만의 특징이 두드러지지는 않는다. 전통사상에 기반한 의철학과 의료윤리 연구가 보강된다면 일본의 연구는 더욱 일본인의 생로병사와 의료를 위한 것이 될 수 있을 것이다. 일본에서는 의학의 발달과 함께 앞으로도 인간강화, 유전자 정보의 수집 및 활용을 다루는 연구가 심화될 것이다. 코로나19 팬데믹의 경험은 인간의 삶과 죽음, 고통에 대한 의철학적 연구와 방역과 보건의료인의 역할, 의료자원분배의 문제 등 의료윤리의 논의를 심화시키는 계기가 될 것이다. 또한 온정주의와 공동체의 결정을 중시하는 것으로부터 개인의 자율성을 옹호하는 방향으로 나아가던 일본 의료윤리 연구는 모두의 건강을 위한 방역과 개인 자유의 충돌이라는 문제에 직면하여 다시 고민에 빠지리라 예상된다.

In this paper, I examine the trends of studies on the philosophy of medicine and medical ethics over the past decade in Japan through the activities and studies of The Japanese Association for Philosophical and Ethical Researches in Medicine. First, the development of highly advanced medicine has led to the emergence of studies that deal with human enhancement, regenerative medicine, precision medicine, and neuroethics. Second, educational programs on philosophy and medical ethics at medical colleges have attracted attention and produced important research. Third, studies on classic themes such as beginning and end of life issues, self-determination, autonomy, and informed consent have been conducted. Fourth, events and disasters occurred in Japan led to the revitalization of research on related topics. However, Japanese studies generally follow theories and discussions in the West, particularly in the Anglo-American region, so its unique characteristics are not salient. Studies on emerging topics of the development of highly advanced medicine—such as human enhancement and the collection and utilization of genetic information—will continue to intensify in Japan. Studies on the COVID-19 pandemic has yet to emerge, but it is predicted that several pandemic-related studies, including the quarantine and the distribution of medical resources will be published in the future.
