Study on the Effect of CBS Combined with On-Site Observation Teaching in Cardiopulmonary Resuscitation Skill Teaching in Resident Doctors Standardized Training

目的:探讨以病例为引导(Case-based study, CBS)联合现场观摩教学对参与住院医师规范化培训心肺复苏技能教学的效果及满意度。方法 选取重庆市急救中心参与2019年住院医师规范化培训的120名学员, 按照数字随机法分为传统教学组(n=60)和CBS联合现场观摩教学组(n=60), 比较两组学员在培训后的理论成绩、心肺复苏操作成绩、电除颤成绩、气管插管成绩、学习风格和满意度。 结果:在培训结束后, CBS联合现场观摩教学组中学员的理论成绩、心肺复苏操作成绩、电除颤成绩和气管插管成绩均明显高于传统教学组(P均<0.05);CBS联合现场观摩教学组中竞争型、协作型和独立型格拉沙-赖克曼的学习风格量表(Grasha-Reichmann Student Learning Style Scale, GRSLSS)分数明显高于传统教学组(P均<0.05) ;依赖型GRSLSS分数在CBS联合现场观摩教学组中明显低于传统教学组(P <0.05);CBS联合现场观摩教学组中学员的满意度明显高于传统教学组(P <0.05)。结论:在住院医师规范化培训心肺复苏技能教学中采用CBS联合现场观摩教学不仅能够提高学员的考试成绩, 还能够影响学员的学习风格, 值得在住院医生规范化培训中进行推广。
objective: To investigate the effect and satisfaction of CPR skill teaching in resident physicians standardized training by case-based study (CBS) combined with on-site observation teaching. Methods 120 students who participated in the standardized training for resident doctors in Chongqing Emergency Center in 2019 were randomly divided into traditional teaching group (n=60) and CBS joint on-site observation group (n=60) according to the random number method. After training, the two groups of students were compared in terms of theoretical performance, performance of cardiopulmonary resuscitation, electrodefibrillation, tracheal intubation, learning style and satisfaction. Results: After the training, the theoretical scores, cardiopulmonary resuscitation operation scores, electrical defibrillation scores and tracheal intubation scores of students in the CBS combined with on-site observation group were significantly higher than those in the traditional teaching group (P <0.05). The scores of competitive, cooperative and independent Grasha-Reichmann Student Learning Style Scale (GRSLSS) in CBS group were significantly higher than those in traditional group (ALL P <0.05). The scores of dependent GRSLSS in CBS group were significantly lower than those in traditional group (P <0.05). The degree of satisfaction of students in CBS joint observation group was significantly higher than that in traditional teaching group (P <0.05). Conclusion: CBS combined with on-site observation teaching in cardiopulmonary resuscitation skill teaching in standardized training for residents can not only improve students test scores, but also affect students learning style, which is worth promoting in standardized training for residents.
