심리부검(psychological autopsy) 기반 알코올 사용장애 자살유형 분류

Typology of Suicide by Alcoholic: Based on Psychological Autopsy

본 연구는 알코올 사용 장애 환자들의 자살 하위유형을 탐색하기 위해 실시한 연구이다. 알코올은 다양한 연구에서 자살에 영향을 미치는 요인으로 중요하게 연구된 만큼 일반적인 성인을 대상으로 한 기존의 연구들과는 다르게 알코올 사용 장애 환자들에 대하여 구체적인 자살 하위유형을 분류할 필요가 있다. 따라서 본 연구는 총 26명의 자살 시도를 한 알코올 사용 장애 환자를 대상으로 심리부검을 활용해 자살의 하위유형을 분류했다. 잠재계층분석을 활용한 연구의 결과 만성적 스트레스 요인을 가지고 있는 만성적 유형, 급성적인 스트레스 요인만을 가지고 있는 급성적 유형, 만성적 스트레스 요인과 급성적 스트레스 요인 모두를 가지고 있는 복합요인 유형으로 분류됐다. 추가적으로 각 유형에 속하는 환자들이 가지고 있는 인구통계학적 정보와 자살관련 요인, 스트레스 요인에 대한 분석을 통해 유형들의 특징을 보다 구체적으로 살펴보고자 하였다. 본 연구는 알코올 사용 장애 환자들의 자살 하위유형을 탐색함으로써 자살 위험을 가지고 있는 알코올 사용 장애 환자들에 대하여 보다 효율적인 치료 방법을 마련할 수 있는 이론적 기반을 제시하고자 하였으며 알코올 사용 장애 환자들의 자살을 이해하는 데 도움을 주고자 했다.

Current study conducted to explore subtypes of suicide by alcoholic. Alcohol was studied as factor affecting suicide in many research, so it is needed to explore specific subtype by alcoholic different from other studies in adult. Therefore, current study classified suicide subtypes by alcohol dependence based on psychological autopsy. Result in study with latent class analysis, the subtypes classified with Chronic (who have chronic stress factor), Acute (who have acute stress factor), Hybrid (who have both stress factor). In addition, current study presented demographic information in subtypes patient and conducts cross-analysis with variables related with suicide and stress factor to explore specific characteristic in subtypes. Current study has some limitation in conducting study but, current study presents theoretical base about more effective therapy method to alcoholic by exploring suicide subtypes by alcoholic. Also, current study help understand about suicide by alcoholic.
