암환자를 돌보는 간호사의 도덕적 민감성과 도덕적 고뇌가 직무만족에 미치는 영향

The Effect of Moral Sensitivity and Moral Distress of Nurses in Care of Cancer Patients on Job Satisfaction

본 연구는 암환자를 돌보는 간호사를 대상으로 도덕적 민감성과 도덕적 고뇌, 직무만족을 파악하고, 직무만족에 영향을 미치는 요인을 규명하여 암환자를 돌보는 간호사의 직무만족을 증진시킬 수 있는 근거자료를 제공하고자 한다. 본 연구의 대상은 서울, 경기, 전남소재 대학병원의 암 병동 및 외래에서 근무하는 간호사 128명을 대상으로 하였고, 자료수집 기간은 2020년 4월 1일부터 29일까지였다. 수집된 자료는 SPSS/WIN 25.0프로그램을 사용하여 기술통계, t-검정, ANOVA, Pearson 상관계수 및 다중회귀분석을 실시하였다. 분석결과 도덕적 민감성은 7점 만점에 4.69±.61점, 도덕적 고뇌는 총 366점 만점에 99.20±48.87점, 직무만족은 5점 만점에 3.45±.55점으로 나타났다. 직무만족은 도덕적 민감성(r=.292, p<.001)과 정적 상관관계, 도덕적 고뇌(r=-.285, p<.001)와 부적 상관관계로 나타났다. 직무만족에 영향을 미치는 요인은 도덕적 고뇌(β=-.385, p<.001), 총 임상경력(β=.374, p<.001), 도덕적 민감성(β=.244, p=.002)의 순으로 나타났으며 이들 변인의 설명력은 31.5%이었다(F=20.44, p<.001). 이상의 결과를 바탕으로 암환자를 돌보는 간호사의 도덕적 민감성을 높이고 도덕적 고뇌를 감소시킬 수 있는 프로그램의 적용으로 직무만족을 높일 수 있을 것이다.

The purpose of this study were to identify the moral sensitivity and moral distress of nurses who take care of cancer patients, and identified influencing factors on job satisfaction of nurses who take care of cancer patients. A descriptive survey was conducted with 128 nurses working in cancer wards and outpatient department at university hospitals in Seoul, Gyeonggi-do and Jeonnam. The data were collected from April 1 to 29, 2020 and analyzed using percentage, frequency analysis, average, standard deviation, t-test, ANOVA, Pearson correlation coefficient and stepwise multiple linear regression analysis, with SPSS/WIN 25.0. The factors affecting job satisfaction are total of nursing experience (β=.374, p=.000, moral sensitivity (β=.244, p=.002), moral distress (β=-.385, p=.000). These variables explained 31.5% of the variance. Findings indicate that moral distress, total of nursing experience and moral sensitivityare important variables influencing job satisfaction of nurses who take care of cancer patients. Development of programs that can increase moral sensitivity and reduce moral distress will increase the job satisfaction of nurses who are in care of cancer patients.
